This patient-centered, unit-based model of care integrates pharmacists with responsibility for both the clinical and the distributive activities of the pharmacy department. Pharmacists are actively engaged in medication management in coordination with physicians and the interdisciplinary team and exhibit a high degree of ownership and accountability for the medication-use process.

Responsibilities

A day in the life of a Unit Based Pharmacist at Hackensack Meridian Health includes:

* Reconcile medications upon admission for admitted patients
* Reconcile medications for patients based on criteria and patient needs prior to discharge to ensure continuity of care
* Develop individualized treatment and monitoring plans for patients
* Review and validate all orders prior to first dose administered
* Monitor patient medication profiles daily
* Participate in patient care rounds when applicable
* Educate patients about new medications
* Counsel patients who are prescribed high-risk medications or complex medication regimens on their discharge medications during inpatient hospitalization.
* Identify barriers to patient medication adherence and work with the interdisciplinary team to address these issues prior to discharge.
* Cover assigned shifts as determined by departmental need, including evenings, weekends and holidays.

Qualifications

* A minimum of a Bachelor of Science in Pharmacy from an accredited college of Pharmacy. Pharm D. strongly preferred
* Valid NJ Pharmacy license is required
* New graduates who have residency experience, preferably in a hospital or acute care environment, are welcome to apply
* Individual must possess strong interpersonal skills and work well at the team level, as well as independently
* BLS/ACLS preferred
* PGY1 Pharmacy Residency strongly preferred
* PGY2 Pharmacy Residency in ER preferred
* BPS recognized certification preferred as well as a minimum of 2 years ER experience
