Description
As an Transplant Clinical Pharmacy Specialist at Children's Health, you will play an important part in helping children get the care they need.

The pharmacist is responsible for all pharmaceutical services in assigned areas. As a member of the multidisciplinary healthcare team the clinical pharmacist promotes an environment of safe, rational, effective and efficient medication use while providing services to meet the needs of the patient.

The Transplant Clinical Pharmacy Specialist supports the heart, liver, and kidney transplant services. Moreover, the Transplant Clinical Pharmacy Specialist is responsible for ensuring the delivery of safe, efficient, and cost-effective immunosuppression drug therapy across Children's Medical Center (CMC) health-system.

This is a full-time position, Monday- Friday 8:00am- 5:00 pm.

Highlighted Responsibilities:

* Manages pharmacotherapy for transplant patients
* Educates and trains pharmacy staff and other health professionals
* Assists in the development of transplant program specific protocols and guidelines
* Research for publication in peer-reviewed literature
* Precepts pharmacy students and residents as well as medical residents and fellows
* Presents at local, regional, state, and national conferences
* Assessing immunosuppression therapy to ensure optimization and improve patient outcomes
* Creating and implementing guidelines and policies for optimal medication use
* Managing drug formulary specific to immunosuppressive agents
* Monitoring pharmacokinetic parameters of immunosuppressive agents
* Participating in quality assurance programs to enhance transplant pharmaceutical care
* Evaluating drug therapy impacted by the selection of immunosuppressive agents
* Attending multidisciplinary rounds and meetings for heart, liver, and kidney transplant services
* Documenting transplant pharmacotherapy plans within the electronic health record
* Participating in the evaluation of patients for potential heart, liver, and/or kidney transplant

Experience

* At least 1 year of experience of hospital pharmacy practice

Education

* Four-year Degree (B.S. or Pharm.D) from an accredited school of pharmacy required.

Licenses and Certifications

* Current Texas Pharmacist license, required.
* BLS Certification, required.
* Residency training or equivalent experience in both transplant as well as pediatrics
* Board Certified Transplant Pharmacist (BCTXP) within two years of hire
