JOB SUMMARY:
The Transplant Pharmacist is assigned the primary responsibility of providing comprehensive pharmacotherapy care and interventions to transplant recipients and their families in a culturally competent manner. The Transplant Pharmacist works in collaboration with the other members of the multidisciplinary transplant team.
ESSENTIAL FUNCTIONS:
The Transplant Pharmacist provides and documents pre-, peri-, and post-transplant related pharmacy services to the solid organ transplant programs
* Pre-Transplant Phase
* Educates potential transplant recipients and their family/support person(s) on transplant medications and adherence to medication regimen;
* Assesses patient/family eligibility to comply with post-transplant medication regimen and communicates findings to multidisciplinary transplant team;
* Assists in determining living donor eligibility based on patients current medication history;
* Serves as a pharmacy resource for other members of the transplant team;
* Screens for patient eligibility for enrollment into research studies as applicable
* Peri-operative Transplant Phase
* Educates transplant recipients and their family members on transplant medications and adherence to medication regimen;
* Serves as liaison between transplant patient and members of transplant team regarding medications and medication-related issues;
* Identifies and works to resolve medication related issues (financial, logistic, etc.) for transplant recipients and living organ donors;
* Actively participates in discharge planning for all transplant recipients and living organ donors;
* Serves as pharmacy resource for other members of the transplant team
* Post-Transplant Phase
* Provides on-going evaluation of transplant recipient medication regimen;
* Communicates transplant recipient medication-related issues and concerns to appropriate members of the transplant team;
* Collaborates with other members of the transplant team on meeting goals of individual patient comprehensive care plans
MARGINAL FUNCTIONS:
* Participates in Transplant Center Program meetings, projects and events;
* Participates in Transplant Department Quality Assurance/Performance Improvement initiatives;
* Maintain involvement in professional organizations;
* Review and maintain currency of related professional literature;
* Performs drug information searches, utilizing various computerized databases;
* Participates in clinically related research studies;
* Provides in-services to hospital nurses and home health agencies about medications where applicable;
* Perform other related duties as assigned
PHYSICAL REQUIREMENTS:
Fast-paced clinic and inpatient setting. Involves standing, walking, bending and sitting.
EDUCATION, TRAINING, AND EXPERIENCE:
* Graduate from an accredited School of Pharmacy with a Doctorate of Pharmacy (PharmD.) degree
* Completion of a specialty residency or fellowship in solid organ transplantation or documentation of equivalent experience
* Active licensure with the North Carolina Board of Pharmacy
