Purpose
The Patient Care Coordinator is responsible for performing intake activities and functions in accordance with company policies and procedures to assist in the flow of dispensing drugs to the patient.

Shift Details: Monday-Friday 9:00am - 6:00pm

Essential Duties and Responsibilities

  • Communicate with providers for prescription renewals or prescriptions for change in dose
  • Updating concise progress notes to ensure other roles within the workflow are able to quickly assess the status of the refill
  • Enter new provider information (i.e., National Provider Identifier) accurately into the system so that the Business Intelligence group can complete their reporting
  • Communicate with patients to obtain new prescriber information
  • Perform other duties as assigned


Minimum Requirements:

  • High School Diploma or GED
  • Current CPhT (Certified Pharmacy Technician) certification required; must be current, without restrictions and in good standing
  • Prior experience working in a Pharmacy setting required

Critical Requirements:

  • Prior experience working in a Pharmacy setting required
  • Effective communicator
  • Excellent organizational and time management skills
  • Competent in basic computer skills
  • Multi-task oriented
  • Ability to effectively work with people
  • Conscientious and detail oriented
  • Flexible and adaptable
  • High School Diploma required

Preferred Requirements:

  • Healthcare experience preferred
  • 2+years pharmacy experience preferred

Education Requirements:

  • High School Diploma or GED
