Job Description

Serves as a pharmacist and subject matter expert within a specialty area, [e.g., Natural Medicines/Dietary Supplements, Ambulatory Care, or other specialty (as determined by senior pharmacy management at time of job assignment)] required for the provision of clinical support to patients, caregivers, and health care professionals.

Job Responsibility

* Applies specialized knowledge of clinical use(s) including mechanism of action, dosing, adverse effects, and drug interactions related to performing patient care activities in collaboration with other members of the inter-professional team.
* Provides accurate, appropriate, and timely clinical consultation services to patients, caregivers, and health care professionals.
* Recommends evidence-based regimens and monitoring plans.
* Consults with physicians and other health care providers in selecting an appropriate regimen to address patients' needs and overall therapy goals.
* Supports efforts of inter-professional team to educate patients and caregivers about non-pharmacologic measures (e.g., diet, exercise, immunization) that promote health and well-being.
* Facilitates development and implementation of treatment guidelines, protocols, and critical pathways.
* Implements educational, training and competency programs for members of the inter-professional team.
* Serves as a preceptor to pharmacy residents and students.
* Participates in developing and implementing related new programs to meet departmental goals and improve services.
* Prepares and dispenses medications according to established policies, procedures, and protocols.
* Ensures safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols.
* Contributes to quality and effective operation of the department.
* Documents clinical interventions in accordance with policies and procedures.
* Performs related duties, as required

Job Qualification

* Bachelors, Masters, or Doctorate Degree in Pharmacology, Pharmacy Science or related field recognized by the state to practice as a licensed pharmacist in New York.
* Current registration and license in good standing to practice as a pharmacist in New York State, required.
* Minimum of three (3) years of related work experience (including PGY residency experience), preferred.
* Training or certification related to assigned specialty, preferred.
* ASHP-accredited post-graduate residency training, preferred.
