**Position description**

Provides pharmaceutical care services in assigned clinic areas. Designs, implements, monitors, evaluates and modifies patient pharmacotherapy to ensure that medication is indicated, effective, safe and convenient. Completes documentation and billing processes for pharmacist activities. Participates in collaborative practice and/or protocol-driven practice, as appropriate. Participates in educational and scholarly activities and educates healthcare professionals, students and patients regarding rational drug therapy. Participates in quality improvement and resource management initiatives. Participates in and supports research activities of the department and the clinic areas served. Supports the continuous improvement, patient safety, and various accreditation programs of the department and institution. Provides support to other departmental functions and initiatives.

**Qualifications**

Graduate of an accredited College of Pharmacy. Doctor of Pharmacy (Pharm.D.) degree or equivalent experience. Equivalent experience includes at least two years of relevant pharmacy practice experience. Computer experience. Commitment to excellence, accuracy, attention to detail and team work. Ability to communicate effectively, work collaboratively with others, organize time well, solve problems, and work independently with minimal supervision.

**Additional qualifications**

The following are preferred: Experience in providing progressive ambulatory patient care services. Completion of an accredited pharmacy residency in primary care, ambulatory care, internal medicine, or geriatrics. History of scholarly activities including publication and scientific meeting presentations. Experience and /or interest in teaching.

**License or certification**

License to practice Pharmacy in the State of Minnesota or eligibility for licensure.
