Description
Delivers pharmaceutical care to oncology patients in both the inpatient and infusion clinic setting through the provision of patient-centered clinical service, therapeutic medication monitoring, protocol management, medication information, education and performance improvement that ensures safe, effective, and cost efficient medication therapy. Under the direction of the Pharmacy Clinical Coordinator, the Oncology Senior Pharmacist works collaboratively with hematology and oncology physicians and nursing staff to provide quality patient care appropriate for the patient's oncologic diagnosis, prescribed treatment, age group and other identified needs. The pharmacist provides comprehensive pharmaceutical care to hematology / oncology patients to assure safe and effective drug therapy. The pharmacist coordinates the drug therapy process on hematology/oncology patient population which may include any of all of the following activities: drug selection, drug information, dosing, monitoring, outcomes management and patient education/counseling. Provides support for the development of the Oncology module in the Electronic Health Record. Participates in the Bone Marrow Transplant Steering Committee and Palliative Care Team. Mentors Pharmacists in clinical patient services. Also functions as a staff Pharmacist.

Education, Certification, and Licensure Qualifications:

* Graduate from a college of Pharmacy accredited by the American Council on Pharmaceutical Education, required.
* Doctorate of Pharmacy (PharmD) required.
* Completion of Post Graduate Post Graduate Year 2 (PGY2) Oncology Residency or Board Certified Oncology Pharmacist (BCOP) certification required.
* State of California Pharmacist License required.

Experience, Knowledge, and Skills Qualifications:

* Previous experience in an appropriate setting (e.g. as a hospital / acute care oncology pharmacist) is desired.
* Experience training and mentoring others is desired.
* Medication Use Evaluation Procedures, Therapeutic Medication Monitoring (including antibiotic, anti-seizure and immune suppressants) and quality improvement process.
* In-depth knowledge of patient / healthcare information systems.
* Knowledge of current trends and issues in adult and pediatric oncology pharmaceutical care.
* Requires an in depth knowledge and the ability to apply legal requirements and accreditation standards including TJC (The Joint Commission), Title XXII, DHS, DEA, FDA, FACT and USP.
* Requires critical thinking skills, decisive judgment, delegation skills and the ability to work with minimal supervision.
* Works in a complex and dynamic environment and takes appropriate actions in emergent situations.
* Communicates in an effective, tactful and diplomatic manner.
* Demonstrates leadership skills and supervisory abilities.
* Must have communication and project management skills.
* Must have the skills to produce accurate work results within established timeframes.
* Ability to communicate verbally and be understood in person or on the telephone.

* Frequent repetitive motions of hands and arms.
