Provides pharmaceutical services to long term care and institutionalized patients in a manner that maximises quality and patient safety.  Participates in and oversees the dispensing of medication in accordance with federal and state regulations, as well as oversees pharmacy operations as a whole.


Essential Funcations:

  • Maintains patient database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including over-the-counter and home remedies).
  • Collaborates with the pharmacy and regional staff to provide clinical and technical support as needed.
  • Documents all communication with physicians, nurses, other members of the PharMerica team and patients and their family members in the pharmacy system.
  • Obtains, as necessary, and maintains all required documentation in accordance with federal and state regulatory agencies and PharMerica corporate compliance policy.
  • Implements policies and procedures for the safe procurement, storage, distribution and use of drugs and biological, as well as disposal of drugs.
  • Works with pharmacy management to ensure implementation of daily operations and implements quality and cost containment strategies.
  • Directs staff on scheduling, purchasing, clinical matters and other tasks.
  • Performs other tasks as assigned.

Position Qualifications:

  • Required:  Bachelor’s Degree in Pharmacy
  • Desired:  Pharm D preferred
  • Desired:  One year experience in long-term care preferred
  • Required:  Knowledge of pharmacy laws and federal and state regulations concerning long-term care and pharmaceutical care practices for long-term care residents.  Ability to work cooperatively as a member of a team.
  • Required:  Current license in the state of practice
  • Must be available to work Mon – Fri between the hours of 10:00am - 8:30pm, rotaing weekends.
