Job Summary

The Post-Acute Care Quality Program Manager will work closely with leadership to provide direction in identifying performance improvement opportunities within the Post-Acute Care network of facilities including Home Health, LTC, SNF, Rehab and Hospice.  This is accomplished through quality assessment, prioritzation of opportunities, development and initiatiation of interventions and improvement actions and ongoing monitoring for impact to create a high performing, evidence-based Post-Acute Care network that improves patient safety and clinical outcomes.  Develops and maintains key relationships within the Post-Acute Care network, MyHFN, PHQC, Hospital and Physician Leadership.  Works in collaboration with MyHFN, PHQC and other affliliate leaders to ensure the development of a high quality, comprehensive, patient-centered Post-Acute Care network for effective transitions of care. 

Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Education

Bachlor's Degree - Nursing or health related field

Minimum Experience

5 years - Quality Management and Performance improvement experience with data collection, trending and analysis in the post-acute care setting, including care management across the continuum of care.

Other Required Experience

Nursing Home Administrators License (Lawson Code NHA) - Preferred

Ability to accept responsibility, analyze problems, evaluate reports, and make decisions and recommendations - Required

Possess knowledge of related outside regulatory agencies, roles and responsibilities and knowledge of procedures and processes specific  to patient care and the the principles of performance  improvement - Required

Excellent facilitation and negotiation skills, problem solving, collaboration and program development.  Professional knowledge of PI and quality improvement, data collection, analysis and trending skills - Required
