At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health, and other employed team members.

Support the Pharmacist and NC market by assisting with medication therapy management, patient safety, and quality improvement initiatives. This position will be responsible for the implementation and tracking of pharmacist medication optimization recommendations, triaging incoming pharmacy tasks and referrals, and executing pharmacy reporting. The pharmacy technician will serve as a subject matter expert on Health Plan formularies and pharmacy benefits and will serve as an additional resource to support the market medication and supply system.

Schedule will be Monday-Friday, 8am-5pm.

Primary Responsibilities:

  • Contact community providers and prescribers via fax and phone regarding pharmacist recommendations, track status, triage NC pharmacy fax inbox for responses
  • Follow up with Landmark and community providers regarding proposed medication changes
  • Triage incoming Landmark Pharmacy tasks and referrals and track status
  • Contact community pharmacies to verify co-pays and/or to investigate medication access issues on behalf of patients
  • Educate and provide support to the market by answering questions about Health Plan formularies and pharmacy benefit packages including but not limited to prior authorizations process, B vs D determinations, Medicare prescription drug coverage determination process
  • Provide support and guidance to patients and the market on medication adherence by actively communicating pill packaging, home delivery, and mail order solutions
  • Execute pharmacy claims and/or quality reports as needed to identify opportunities for Pharmacist consult
  • Actively support market with performance/quality issues on medication inventory and McKesson Orbits
  • Ensure market compliance by verifying market is ordering Landmark approved medications/supplies and are managing inventory appropriately in McKesson Orbitz and following appropriate point of care medication administration procedures
  • Provide support to the Pharmacist and the market during annual flu campaign
  • Provide support to the Pharmacist and market on important operational and clinical tasks including but not limited to: care coordination, telephonic campaigns and surveys, transitions of care, medication therapy management (MTM), collection of clinical data and community provider records, market-based pilots
  • Actively participates in the implementation of performance/quality improvement, patient engagement, and patient safety activities with the Landmark Team

Required Qualifications:

  • High School diploma or equivalent
  • Basic knowledge of Microsoft Office Suite
  • Full COVID-19 vaccination is an essential requirement of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. UnitedHealth Group will adhere to all federal, state and local regulations as well as all client requirements and will obtain necessary proof of vaccination, and boosters when applicable, prior to employment to ensure compliance

Preferred Qualifications:

  • Active/current National Pharmacy Technician Certification (PTCB, NCCT, or ExCPT)
  • 3+ years of current/recent pharmacy or PBM experience
  • Geriatric or Medicare Part D experience
  • Ability to work in a fast-paced environment that is constantly changing
  • Excellent telephonic customer service

Physical Demands:

  • Regular requirement to sit; use hands to touch, handle or feel
  • Regular requirement to talk and listen
  • Occasional requirement to stand; walk and reach with hands and arms
  • Occasional requirement to lift and/or move up to 10 pounds
  • Vision abilities include close vision, peripheral vision, depth perception and ability to adjust focus

Work Environment:

  • Will travel as needed to both market office locations, Durham and Winston-Salem
  • Noise level is usually moderate
  • Expected to work normal business hours between 8:30am-5pm Monday - Friday from the Durham office
