Description:

We are looking for experienced pharmacy technicians.

General Summary:

Assists transition patients by evaluating their self-care needs and providing the items to them after a post-acute stay. Assembles and reviews medication history and document their findings.

Assists pharmacists in preparing and distributing drugs to all patients and areas of hospital, maintaining drug inventory, and charging and maintaining patient records. Advanced experience and training in areas of controlled substances, IV admixtures, and handling and preparing cytotoxic drugs.

+ Must have excellent communication and customer service skills

+ Outpatient experience and knowledge of medication indications

+ Transitions of Care Pharmacy Technician is responsible for assisting the pharmacist with medication reconciliation tasks by interviewing patients, obtaining and updating the medication history, and documenting it in the electronic medication record.

+ Assemble medication histories by consulting patient, family members, outside pharmacies and medical records. Document medication histories and communicate findings with pharmacy and clinical staff.

+ Communicates patient feedback and requests to post-acute service team.

+ Coordinates meetings, managing and delivery of the patient's self-care items.

+ Track and monitor patient status and pending transitions to post-acute care.

+ Engage patients to ensure that self-care needs are adequately met including medication, products and services. Reviews medical records to evaluate the patient's self-care needs.

+ Works with clinical team, case management and other medical staff to achieve patient's self-care goals.

+ All duties are performed independently by the technician under the supervision of a pharmacist.

The employee performs job duties with sensitivity to the patient's diverse cultural backgrounds. The employee maintains competency skills to differentiate the dose ranges and dosage forms with consideration to the age of the population served, if applicable.

Pharmacy technicians work only under supervision of registered pharmacists - they do not perform duties that legally canonly be performed by registered pharmacists. Performance demonstrates efforts to improve patient satisfaction, lower costs and improve quality. Supports Mission ofTrinity Health and Holy Cross Health.

Minimum Licensure & Certification Required (if applicable):

1. National pharmacy technician certification, or actively enrolled in Accreditation Council for Pharmacy Education (ACPE) approved pharmacy school program (successful completion of foreign pharmacy graduate examination will be considered equivalent).

2. Registration with MarylandBoard of Pharmacy.

Minimum Knowledge, Skills & Abilities Required:

1. High school diploma or equivalent.

2. Pass math/pharmaceutical calculation test.

3. One (1)year of IV admixture and chemotherapy experience preferred.

4. Able to communicate effectively, and speak, write and comprehend English.

5. Able to work cooperatively with hospital and pharmacy staff, in order to meet objectives.

6. Must be able to handle frequent interruptions, and changes in workload and work schedule.
