Summary The Robert J. Dole VAMC is currently recruiting a Primary Care Clinical Pharmacy Program Manager and is responsible for providing leadership, direction, organization, planning and implementation of clinical pharmacy programs for the Patient Aligned Care Team and Anticoagulation services. Responsibilities The Supervising Pharmacist Program Manager in Primary Care is accountable to the Associate Chief of Pharmacy for providing leadership, direction, organization, planning, and implementation of pharmacy programs which promote stewardship, patient-centered care, patient safety, and optimization of evidenced based pharmaceutical care outcomes. The incumbent is responsible for the development, implementation, monitoring, evaluation, and reporting of the facility's Primary Care Pharmacy programs for the Patient Aligned Care Teams (PACT) and Anticoagulation services. The primary responsibility of the Primary Care Clinical Program Manager is to assume supervisory responsibility for the administration and operations of the clinical pharmacy services in PACT and Anticoagulation. Participates in all Pharmacy Service activities including clinically oriented programs, regulatory and quality assurance activities, and coordinates all functions and activities within the section to ensure they coincide with other sections and the overall goals and mission of service. The supervisor responsibilities include integration of pharmacy services with the Primary Care Division and direct oversight of the Anticoagulation program and is expected to maintain a clinical practice for a portion of their time. In addition, this position will incorporate management oversight of the PGY1 Residency Program, where applicable. Program Management/Administrative Responsible for assessing the current clinical pharmacy programs in Primary Care and establish expansion of clinical programs based on the need of the Medical Center. Perform as a Pharmacy Service leader in all aspects of his/her practice, and build coalitions with other services (Primary Care division, HBPC, Mental Health, Education, etc.) Effectively organizes workload, directs clinical activities, plans short and long-range objectives/goals, solves problems, and achieves established goals within budget and staffing constraints. Assists with the effective utilization of resources and budget to support the mission and goals of the organization. Performs supervisory functions for personnel as identified in the pharmacy organizational chart including, but not limited to, leave request reviews, performance appraisals, disciplinary actions, rewards, and recognition, etc. Clinical Practice Managing a clinical practice within area of expertise which may include, but is not limited to, reviewing consults, providing direct patient care, and assuring coverage for the clinical services when on leave. Evaluates patients and monitors response to drug therapy. Make changes in drug therapy within scope of practice as needed depending on assessment of appropriate laboratory and clinical monitors. Advises provider on aspects of clinical pharmacokinetics as it pertains to the monitoring of serum plasma concentrations and its relationship to patient response. Serves as a drug information resource for health care providers by answering inquiries regarding therapeutics, drug interactions, dosing, side effects, etc. Education Proactively provides ongoing communication related to the pharmacy programs under their supervision to professional staff via Pharmacy and Therapeutics meetings, Medical Staff meetings, Pharmacy Staff Meetings, and individualized meetings as appropriate. Remains up to date about modern technology, management practices, and clinical practice by reading relevant material in reputable journals and resources. Promotes innovation and quality improvement within the Pharmacy Service and facility. Hiring Responsibility Works with servicing Human Resources Management office in participating in all steps of the recruiting and hiring process to expeditiously recruit for vacant positions and to select qualified employees, including obtaining at least two references for the selectee. Successfully transitions new hires into the position by promptly communicating performance elements and standards within applicable timeframes, providing ongoing feedback, coaching, and making appropriate use of the probationary period to assess the employee's ability to perform. Appropriately identifies and addresses specific, performance-related deficiencies with the employee in a timely manner to ensure employee is given every opportunity to improve to a successful level of performance. Develops the ability of the subordinate staff and others, as appropriate, to perform and contribute to the organization through coaching, mentoring, and providing ongoing feedback and by granting opportunities to learn through formal and informal methods. *Other duties as assigned by functional statement. Licensure: Possess a full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the U.S. (i.e. Puerto Rico), or the District of Columbia. Education: Be a graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (TRANSCRIPT REQUIRED) Preferred Experience: Candidates who have completed an American Society of Health-System Pharmacists (ASHP) accredited residency in specialty and/or general pharmacy practice, and/or obtained a board certification in pharmacy practice (i.e., BCPS, BCGP, BCACP, etc.). Board Certification must be obtained through a national organization in which the quality of the certification is recognized by the Clinical Pharmacy Scope of Practice Review Board and the Pharmacy Practice Standards Board. GS-13 Grade Determinations: Experience: One-year of experience equivalent to the next lower grade level (GS-12). Equivalent experience to the next lower grade level (GS-12) may include: Reviewing, interpreting, and verifying medication orders for appropriateness. Processing and filling medication orders, interacting with and making recommendations to other clinical staff regarding medication therapy ordered to ensure safe and effective care. Reviewing the patient's medications, allergies, labs, and other pertinent information from the medical record to identify and solve medication-related problems. Contacting providers as appropriate, documenting recommendations and interventions, providing refill extensions and partial medication supplies. Taking health and medication histories, performing medication reconciliation, providing drug information. Assisting in formulary management including therapeutic substitutions, nonformulary reviews and medication usage evaluations. Documenting and assessing adverse drug events (ADEs), assisting in medical emergencies, providing oversight of technical staff in all aspects of medication distribution. AND You must demonstrate the following Knowledge, Skills, and Abilities (KSA's): Ability to effectively supervise subordinate staff. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards pertaining to pharmacy. Skill in managing people or programs. References: VA Handbook 5005 Part II, Appendix G15, Licensed Pharmacist VA Qualification Standard Physical Requirements: Physical movements involve completion of routine pharmacy functions which include but are not limited to significant computer work/typing, prescription order entry and processing, contact of patients (both face to face and via phone). Certain tasks are repetitive in nature and require multiple steps performed in succession for success. The incumbent should be able to lift and carry up to 30 pounds without significant difficulty. Incumbent must be able to stand or sit in one place for considerable time during completion of complex tasks. Complexity of mental processes involved is extensive, demanding, and challenging.
