Job Summary:

Baptist Health Pharmacy services are committed to ensuring the optimal, safe, and effective use of medications for all patients through advanced technology and innovative application of skilled personnel that will improve clinical outcomes and community health in a value based environment.

The pharmacy residency program is a twelve-month postgraduate training program designed to develop a skilled practitioner who is prepared to deliver patient-centered pharmaceutical services and interact with members of the health care team accountable for the care of patients with complex conditions in a community hospital environment. The resident will also be prepared to pursue postgraduate year two (PGY2) training if so desired. The resident will be responsible for supervising and performing in all pharmacy operations in accordance with accredited hospital policies and procedures, ethical and professional practices, The Joint Commission standards, and State and Federal requirements. The resident will be in compliance with the accreditation standards of American Society of Health-System Pharmacists PGY1 pharmacy residency program.

Principal Duties and Responsibilities:

The following is a summary of the major functions of this individual's job. He or she may perform other duties which are not mentioned below, and specific functions may change from time to time.

1. Interprets, profiles, and dispenses outpatient prescriptions and inpatient orders.

2. Supervises daily production of IV's for scheduled delivery and IV Room restock on 1st shift in a timely fashion.

3. Performs calculations as needed

4. Supervises department or assigned work area operations in absence of

Pharmacy Technician Supervisor, Manager or Director including scheduling appropriate staff coverage when personnel call in sick.

5. Initiates, documents, and participates in department activities that promote positive patient outcomes. These activities may include, but are not limited to:

DUE, ADR reporting, resolving drug therapy questions, therapeutic drug monitoring, identifying drug interactions and allergies, direct contact with prescribers to clarify major problem orders, and non-formulary drug requests.

Performs random checks of outpatient prescriptions, inpatient orders, tech run times, accuracy of pre-packing, Robot, and AcuDose restocking, and other QA checks as assigned and documents QA checks on appropriate forms.

6. Assists in orientation and training of new employees.

7. Helps train for advanced technician programs.

8. Handles patient/staff complaints and computer issues.

9. Monitors and coordinates workflow with established delivery schedules.

10. Coordinates, directs, and reassigns support personnel as appropriate.

11. Participates fully in the Clinical Pharmacy Program at Baptist Hospital Louisville, including, but not limited to:

* Performing interventions with medical staff to achieve optimal drug therapies for disease states
* Promoting effective and efficient conversion of IV to oral therapy for appropriate medications
* Assisting in gathering, trending, and analyzing Drug Utilization Evaluation data
* Promoting appropriate use of anti-infective therapy
* Assisting in the achievement of drug therapy goals as identified by the Pharmacy and Therapeutics Committee
* Assisting in the development of patient and staff education materials and programs
* Assisting in the development of drug therapy protocols based on current best practice standards
* Presenting educational programs to patients and hospital staff
* Promoting appropriate medication dosing regimens
* Promoting preferred formulary therapies to the medical staff
* Providing drug information to medical and hospital staff
* Performing literature searches as needed/requested

12. Communicates with Residency Program Director when needed and in appropriate time frame.

13. Develop a project to be presented at a pharmacy conference

14. Responsible for completion of required residency projects, documents and evaluations in accordance with ASHP standards, goals and objectives.

Minimum Education, Training and Experience Required:

1. Graduate of an ACPE accredited degree program (Bachelor of Science or Doctorate of pharmacy) or eligible for pharmacist licensure

2. Kentucky Pharmacist Registration or eligible.
