I Accountability Objectives:

Post Graduate (PGY1) Pharmacy Residents are provided the opportunity to accelerate their experience beyond entry level pharmacist. They gain professional competence in patient centered care and pharmacy operational services. They develop leadership skills that can be applied to any pharmacist position in any practice setting. There are four required educational and practice experiences in which the resident must receive instructions and competency:

1. Patient Care

2. Advancing Practice and Improving Patient Care

3. Leadership and Management

4. Teaching, Education, and Dissemination of Knowledge

The pharmacy resident provides direct patient care and performs the functions of a pharmacist within multiple pharmacy practice settings.

II Position Qualifications:

Minimum Education, Licensure / Certification and Experience Required.

A. Education

* Doctor of Pharmacy (PharmD) from an Accredited College of Pharmacy.

B. Licensure / Certification

* Valid Controlled Substances License or eligible for a controlled substance license in the State of Michigan within 30 days from date of hire.
* Valid Pharmacist License or eligible for pharmacist licensure in the State of Michigan within 30 days from date of hire.
* Current Basic Life Support (BLS) certification.

C Special Skill / Aptitudes

* Interpersonal skills necessary to communicate effectively with a wide variety of hospital and pharmacy personnel, and patients.
* Demonstrates a commitment to providing outstanding customer service in a manner reflective of the SJMO mission and values as well as the Trinity Health Standards of Appearance, Attitude, Respect, Responsiveness, Communication and Courtesy.
* Demonstrates a commitment to the Department of Pharmacy vision and mission and principles of continuous quality improvement within the organization.
* Excellent customer service orientation skills necessary in order to deal effectively with various levels of hospital personnel, outside customers and community groups.

D. Experience

* Pharmacy experience that includes direct patient care and operational rotations in hospital and ambulatory settings.

III Duties / Responsibilities:

* Completes the competency areas, educational goals, and objectives established for the Pharmacy Residency Program and those specific for each rotation.
* Completes one major longitudinal research project during the residency year and other minor longitudinal projects while on rotation.
* Participates in the education of undergraduate clerkship students and provides educational sessions to pharmacy, nursing and medical staff as appropriate and while on rotation.
* Provides direct pharmaceutical care to the patient populations served; identifies, prevents and resolves drug related problems to promote safe and effective use of medications; monitors patient progress to support positive outcomes; and documents (as required) the appropriate findings, interventions, recommendations, plan/outcomes. Participates in multidisciplinary rounds.
* Provides a safe and effective drug control system; procures, compounds, prepares and dispenses medications according to departmental policies and procedures.
* Provides functional supervision to pharmacy operations in assigned work area. This includes responsibility and accountability for ensuring timely, accurate completion of pharmacy workload during assigned shift. Directly supervises pharmacy technicians in distributive functions and departmental operations
* Participates in continuous quality improvement activities/teams to meet departmental or institutional goals.
* Prepares reports, educates, makes suggestions and recommendations for improvement. Develops protocols, policies and practices.
* Performs other responsibilities and duties as assigned by the residency director or coordinator, or the individual rotational preceptors.
* Provides drug information services to patients and professional staff to assure appropriate use of medications.
* Participates in evaluating medications for formulary consideration, with a pharmacoeconomic analysis and prescribing guidelines, in conjunction with the appropriate specialty preceptor and the P&T Committee.
* Participates in relevant hospital and pharmacy committees as part of the rotational or longitudinal research project activities; establishes collaborative practice relationships with other healthcare providers.
* Participates in local, regional and/or state professional organizations such as ASHP, SMSHP, MSHP and MPA.
* Demonstrates and actively promotes an understanding and commitment to the mission of St. Joseph Mercy Oakland through performing behaviors consistent with the Trinity Health Values.
* Maintains a working knowledge of applicable Federal, State, and Local laws and regulations, the Trinity Health Organizational Integrity Program, including the Standards of Conduct, Code of Ethics, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behaviors.
* Supports and conducts one's self in a manner consistent with customer service expectations.
