Job Description

Provides drug information and assists prescribers in providing optimal pharmaceutical care for patients.

Job Responsibility

* Prepares, dispenses, and manages medications to support safe and effective drug therapy for patients; demonstrates ability to manage formulary and medication-use processes, as applicable to the organization.
* Provides effective medication and practice related education to patients, caregivers, health care professionals, students, and the public; employs appropriate preceptors' roles when engaged in teaching.
* Contributes pharmacy specialist's perspective to an health system's technology and automation systems decisions; contributes to the leadership and management activities within the practice area.
* Exercises practice leadership; evaluates core biomedical literature resources appropriate for the specialized area of pharmacy practice; develops collaborative professional relationships with members of the health care team; for assigned patient caseload, prioritizes delivery of pharmaceutical care.
* Establishes collaborative pharmacist-patient and pharmacist-caregiver relationships; collects and analyzes patient information; triages patients; designs evidence-based therapeutic regimens.
* Employs advanced literature analysis skills in preparing drug information; provides pharmacy expertise to the organization in the review of existing, development of new, and implementation of the health system's policies and procedures affecting the care of patients.
* Identifies opportunities for improving the safety of aspects of the health system's medication-use system.

Job Qualification

* Bachelor of Pharmacy required.
* Current License to practice as a Pharmacist in New York State required, plus specialized certifications as needed.
* 0-1 years of relevant experience, required.
