Description
Lowell General Hospital#s Pharmacy Resident is responsible for preparing, compounding and dispensing medications to patients in accordance within state, federal laws under the guidance of the Residency Program Director and Residency Program Preceptors. The resident is responsible for the optimization of drug therapy by identification, documentation, resolution and prevention of drug related problems. Is able to work independently, within the established policies and procedures of the pharmacy department. Directs and coordinates the pharmacy dispensing and clinical activities of the assigned pharmacy service area. Supervises support staff (when applicable) during an assigned shift. Ensures the pharmacy is operated within the limits of State and Federal regulations Duties # Responsibilities: # Coordinates the delivery of all pharmacy services on the assigned shift, within the limitation of department policies and procedures. Prepare, compound, dispense and deliver medications to patients in accordance with the Board of Registration in Pharmacy, the Department of Public Health, State Law, Federal Law, professional pharmacy practice standards, and department policy in a timely manner. Evaluates medication orders and patient medical records for medication related problems. Documents, and resolves medication related problems. Communicates professionally with Physicians, Nurses and other healthcare professionals regarding medication related issues. Documents competency by routinely documenting clinical pharmacy interventions. Serve as a drug information resource to physicians, nurses, patients and other members of the healthcare team. Maintains skill, knowledge and competency through department initiated trainings, attendance at journal clubs, attendance at external trainings, routinely reviewing medical literature. Performs medication counseling and medication history taking when required, expertly performs basic patient care services such as blood glucose testing, blood pressure monitoring, peak flow meters, BLS/ACLS (if required by care setting). Participate in department meetings, and pharmacist practice council. Orients and trains pharmacy interns, technician trainees, other students and newly hired personnel. Manage support staff including pharmacy technicians, interns, students, and verify the accuracy of their work. Work with the Pharmacy Technician Supervisor to accomplish departmental tasks and realize operational efficiencies. Ability to multitask and work efficiently, which is defined as the ability to verify a minimum of 17.5 orders per hour (while in a staffing role) averaged monthly and providing quality pharmaceutical care to a patient care unit (when assigned to a (clinical role). Pharmaceutical care activities includes, therapeutic drug monitoring, patient counseling, medication reconciliations, attendance on interdisciplinary rounds or other disease state targeted activities.
