Were searching for a Pharmacy Resident the post-graduate year one pharmacy resident is a one year, organized, directed program that builds upon knowledge, skill, attitudes and abilities gained from a professional pharmacy degree program. The first-year residency program enhances general competencies in managing medication-use systems and supports optimal medication therapy outcomes for patients through application of core outcomes and goals. Promotes and participates in the defined missions of the hospital and the pharmacy. Actively promotes the profession of pharmacy by participating in the education of students on clerkships and experiential experiences.

Essential Functions

  • Provide evidence-based, patient-centered medication therapy management with interdisciplinary teams through organized rotations in various patient populations.
  • Collect and analyze patient information
  • Design evidence-based medication regimens, including goals, outcomes and monitoring plan.
  • Evaluates medication plans for appropriate medication selection, dosage, route, frequency, duration of therapy and drug interactions (drug-drug, allergy, drug-disease, drug-lab, drug-food) in relation to the patients clinical status.
  • Performs daily profile review to assess appropriateness of medication therapy in relation to patients ongoing and changing disease state and clinical status.
  • Participate in the improvement of the medication-use process through performance improvement activities, including analysis of adverse drug events and medication-use-evaluations. Design, implement, and monitor at least one medication improvement activity during the year.
  • Prepare and dispense medications following proper preparation, labeling, dispensing and billing.
  • Participate in practice management activities, working with the pharmacy systems manager and pharmacy coordinators to establish specific goals and outcomes desired.
  • Provides information to healthcare professionals and families regarding drug therapy.
  • Answers drug related questions in a timely and professional manner utilizing drug information retrieval skills with assistance from the Drug Information Coordinator as necessary.
  • Provides discharge teaching instructions, including patient information sheets and medication calendars if warranted, and documents in patient chart appropriately.
  • Provide educational in-services to healthcare professionals on a routine basis, including provision of one Nursing Grand Rounds topic and two formal Resident Noon Conferences.
  • Participate in the education of pharmacy students by serving as a preceptor for students while on rotation at Childrens Hospital. Actively participate in patient discussions and formal presentations for pharmacy students during clerkships.
  • Design, development and implement one clinical research project that will benefit the patients at Childrens Hospital and the organization.
KNOWLEDGE, SKILLS AND ABILITIES
  • Mathematical skills necessary for algebraic conversion, subtractions, multiplication, and division.
  • Verbal and written communications skills sufficient/necessary to receive and deliver information accurately among pharmacy staff and others.
EDUCATION AND EXPERIENCE
  • Graduate of an accredited school of pharmacy required.
  • Previous experience in health care area or unit dose medication preparation preferred.

CERTIFICATIONS/LICENSURE REQUIREMENTS

  • Must be a licensed Nebraska pharmacist or be eligible for licensure in the state of Nebraska within 90 days of hire.
  • Successful completion of security check in compliance with requirements set by the State of Nebraska
