Description
In this role, you will provide pharmaceutical and therapeutics management and oversight and support to all Clinic personnel, including but not limited to Infusion, ASC, and Nuclear Medicine. Interfaces with Clinic personnel and provides pharmaceutical consulting and education. Provides pharmaceutical oversight to staff who provide medication mixing and administration of medications, ensuring the Clinics pharmaceutical care complies with all applicable laws. Oversees compliance and all policies and procedures for Infusion, ASC, and Nuclear Medicine.

ESSENTIAL FUNCTIONS and RESPONSIBILITIES:

Provides and supervises delivery of pharmaceutical care in compliance with all federal/state laws and regulations.

* Participates in Infusion Governance Committee in an outpatient infusion center
* Participates in the Pharmacy and Therapeutics Committee of Vancouver Clinic that oversees all medication management within the organization.
* Provides pharmaceutical oversight for medication management in the ASC, Imaging, Infusion, and other high medication use areas of Vancouver Clinic.
* Reviews all therapy plans and associated protocols in the infusion center and provides drug information to ensure evidence based care in the infusion center.
* Oversees and/or performs audits for the administration, inventory and storage of controlled substances in the organization.
* Manages organizational resources with fiscal responsibility to promote cost effective utilization.
* Manages inventory choices for medications used in the organization to minimize cost and waste.
* Participates and partners with providers in drug utilization evaluation.
* Assures appropriate education of the healthcare team concerning drug choice, delivery method, cost, and efficiency.
* Provides drug information and consultations to physicians, nurses, and other health care providers.
* Keeps abreast of current topics in healthcare and pharmacy practice and communicates these to staff and providers.
* Participates in Population Health initiatives and Medicare Part D Formulary management for Risk Based contracts.
* Promotes the use of cost-effective medication regimens where appropriate.
* Participates in continuous quality improvement
* Enhances communication and problem solving processes by identifying opportunities for improvement.
* Provides enhancements to the medication use process

SKILLS AND ABILITIES:

* Ability to consistently demonstrate essential clinical competencies for this role and license.
* Ability to keep up to date with regulatory policies and procedures.
* Ability to instruct and mentor others while working in a multi-disciplinary team.
* Project management and organizational skills.
* Requires exceptional written and verbal communication skills.
* Comfortable engaging with stakeholders at all levels of the organization.
* Ability to monitor inventories and keep accurate records.
* Excellent analytical and problem solving skills.

EDUCATION AND EXPERIENCE:

* PharmD from an accredited Pharmacy program.
* Active unencumbered WA State Pharmacist license.
* Current OR State Pharmacist license, or ability to obtain one within 6 months of hire.
* Residency training and/or five years' hospital or ambulatory pharmacy experience preferred.
* Experience with infusion pharmacy preferred.
* Experience in USP 797/800 oversight preferred.
