Overview

As a member of the interdisciplinary team, provides professional pharmaceutical knowledge in the provision of pharmaceutical care and in the preparation of, dispensing of, and counseling of medications to patients and families. Maintains a practice environment that is in accordance with Iowa Board of Pharmacy Rules, Joint Commission, USP, ASHP, DEA, and the FDA.

    Responsibilities

    Essential Functions/Responsibilities:

    Patient Care

    · Interprets reviews, enters, prepares and/or compounds, verifies, dispenses medication orders.

    · Performs drug utilization review, care coordination, and counseling to all patients.

    · Monitors patient medication therapy with regard to clinical appropriateness including dose, administration schedule, allergies, drug-drug, drug-food interactions and cost considerations

    · Communicates and consults with patients and physicians regarding recommendations and suggestions concerning medication therapy in accordance with departmental practice for the assigned service area

    · Documents clinical, educational and consulting activities according to departmental procedures

    Departmental Support

    · Supervises pharmacy technicians in an efficient and effective manner, consistent with applicable laws, regulations, and policies.

    · Promotes the development, preparation, implementation and attainment of departmental goals, policies, and projects

    · Serves on inter and intra departmental committees and interdisciplinary patient care committees as assigned

    · Assists with patient, departmental, and physician educational programs.

    · Attends in-services, on the job training, and attends at least 75% of required department meetings.

    · Performs departmental inspections and controlled substance verification as assigned

    · Assists with the education, training or orientation of pharmacy students, interns, technicians, and new hires

    Technical Support

    · Exhibits and maintains extensive knowledge of the electronic EMR and the software program to prepare, adjudicate and process prescriptions

    · Exhibits and maintains and extensive knowledge in processing insurance claims and reimbursement procedures

    · Maintains extensive knowledge in the utilization of all automation equipment used in filling, labeling, and dispensing prescriptions.

    · Assists in maintain adequate stock levels of pharmaceuticals and supplies through established inventory procedures. Communicates unusual usage patterns to supervisor and inventory staff.

    · Assists in the management of 340B and Pharmacy virtual and on-hand inventories.

    · Employee maintains current licenses and/or certifications required for the position.

    · Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.

    · Completes all annual education and competency requirements within the calendar year.

    · Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

    Education:

    BS in Pharmacy obtained within the United States

    Experience:

    Completion of retail portion of pharmacy school rotations

    1 to 3 years of retail experience. Does not have to be pharmacy experience

    License(s)/Certification(s):

    Valid Iowa Pharmacist License in Good Standing

    Knowledge/Skills/Abilities:

    Read/Write/Speak fluent English

    Technical aptitude

    Mechanical aptitude

    Basic Computer Skills using word, email, and web browser

    Use of usual and customary equipment used to perform essential functions of the position.
