Providence is calling a Pharmacist Resident - PMMC Pharmacy Residency Program (1.0 FTE, Days) to Providence Medford Medical Center in Medford, OR.

This position is only open to candidates who have gone through the ASHP Phorcas PGY1 Pharmacy Residency Phase 2 process and who have interviewed with us.

Consistent with applicable law, full vaccination for COVID-19 or obtaining an approved exemption is a condition of employment. Medical & religious exemptions can be requested on your first day of work. Medical exemptions require a qualifying medical reason preventing safe vaccination and signed documentation from your medical provider; religious exemptions require sincerely held beliefs. Currently, caregivers with exemptions cannot work in positions with direct patient contact in skilled nursing and assisted living facilities. We are seeking a Pharmacist Resident - PMMC Pharmacy Residency Program that during the one year training program, will be responsible for developing the skills, knowledge and attitude necessary for the provision of high quality pharmaceutical care. As such, they are expected to demonstrate the ability to function effectively as a staff pharmacists, meeting the same standards of performance, duties and functions as are required of the staff pharmacists.

In this position you will have the following responsibilities:

* Review and interpret physicians orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight and comorbidities

* Check for drug incompatibilities and allergies, and take appropriate action when necessary

* Review patient records and orders regarding drug therapy, recommend and initiate changes that are consistent with the patients age; is successful in improving patient outcome through monitoring and intervention consistently

* Regularly consult with physicians and nursing staff members concerning the use, availability, dosage form, side effects and possible incompatibilities of new as well as currently used drugs; always take time to clearly explain information to concerned patients

* Follow proper computer order entry procedures

* Mix IV additive solutions and chemotherapy as required using good aseptic technique within the laminar flow hood

* Be responsible for the completion of all assigned work procedures during the shift while maintaining the highest standards of quality

* Be defined as part of PHS's clinical services (DUE, pharmacokinetics etc.), concurrent interventions and follow-up will be made in order to assure the appropriateness, safety, and efficacy of the medication

* Continually strive to make the best use of time during the assigned shift through careful coordination of daily tasks

* Use pharmacokinetic principles in establishing patient specific dosage regimens as appropriate and effectively document all clinical monitoring

* Understand principles of the hospital formulary and follow formulary procedure

* Maintain current awareness of and comply with the Quality Assurance programs, including awareness of current monitors, and participate as indicated

* Document all clinical interventions and workload statistics

* Complete monthly surveys of assigned nursing units medication rooms to ensure proper drug storage and expiration

* Responsibility of Resident:
* Complete required rotation objectives
* Complete required residency project
* Complete required presentations
* Complete required evaluations
* Complete any additional projects/programs as requested

Required qualifications for this position includes:

* Graduate from an accredited college -OR- university of pharmacy

* If not licensed at the time of initiation of the program, the resident is expected to obtain licensure from the Oregon Board of Pharmacy during the residency year

Preferred qualifications for this position include:

* PharmD
