Providence is calling a Pharmacist Resident (1.0 FTE/Day Shift) to work in the Pharmacy Residency at Providence Centralia Hospital in Centralia, WA

The Pharmacy Practice Resident, during the one-year training program, is responsible for developing the skills, knowledge and attitude necessary for the provision of high quality pharmaceutical care. He/she is expected to demonstrate the ability to function effectively as a staff pharmacist, meeting the same standards of performance, duties and functions.

Performs all duties in a manner which promotes team concept and reflects the Sisters of Providence mission and philosophy.

In this position you will:

* Review and interpret physicians' orders to assure that the drug, dosage form, and dose is appropriate for the patient's age, weight, condition and comorbidities. Check for drug incompatibilities and allergies, and take appropriate action when necessary.
* Review patient records and order regarding drug therapy, recommends and initiates changes that are consistent with the patient's age; is successful in improving patient outcome through monitoring and intervention consistently.
* As required, regularly consult with physicians and nursing staff members concerning the use, availability, dosage form, side effects and possible incompatibilities of new as well as currently used drugs; always take time to clearly explain information to concerned patients.
* Follow proper computer order entry procedures.
* Mix IV additive solutions and chemotherapy as required using good aseptic technique within the laminar flow hood.

Required qualifications for this position include:

* Graduated from an accredited college or university of pharmacy
* Active Washington State Pharmacist license
* Pharmacy Intern License may substitute for Pharmacist within the first 120 days of employment with a licensed Pharmacists' supervision
* Experience in hospital pharmacy practices as a student, intern or pharmacist preferred

Preferred qualifications for this position include:

* PharmD
