Description
The purpose of this position is to collaborate with faculty, staff and patients to improve patients' health. The pharmacist will provide collaborative drug management services, complete medication reviews, medication reconciliations and other clinical pharmacy services in addition to traditional dispensing services and other services as required. The pharmacist will assist medical personnel and patients with drug information. the pharmacist will help provide direction to pharmacy interns, residents, and technicians. This is a relief position and will work a variety of hours as needed.
+ B.S. Pharmacy or Pharm D.
+ Community pharmacy practice
+ Drug therapy in patients described in Position purpose(Dosage, indications, side effects, etc.) Contemporary medication use systems (interpret prescribers' orders, prepare and dispense medications, etc)
+ Excellent interpersonal communication skills
+ Systematic problem solving skills, ability to work independently with minimal supervision.
+ Successfully completes initial and continued competencies as required for maintenance of standard of practice, legal regulatory compliance and OHSU policies.
+ Oregon State Board of Pharmacy License
+ Must be able to perform the essential functions of the position with or without accommodation.
+ Board certification in Ambulatory Care Pharmacy, pharmacotherapy, or Psychiatric Pharmacy
+ Clinic based practice
+ Oregon State Board of Pharmacy Preceptor License
