Overview Seton Medical Center Harker Heights (SMCHH) is an 83-bed acute care hospital opened in 2012 in Harker Heights, Texas offering an array of services such as a Cardiology, Emergency Services and a Level IV Trauma Designated Emergency Room, General Surgery, Orthopedic Surgery, Total Joint Replacement, Gastroenterology, a dedicated Women's Center, Diagnostic Services and more. We provide exceptional care through collaboration with our caregivers, physicians, volunteers and community partners. Our award-winning staff provides patient care with dignity and respect. We are Joint Commission Accredited, an Accredited Chest Pain Center, and Advanced Primary Stroke Center. SMCHH is a joint venture between the Austin-based Ascension and Nashville-based Ardent Health Services. POSITION SUMMARY: This position is responsible for effectively and safely dispensing medications and other pharmaceutical supplies; in order to fill medication orders issued by physicians and other qualified persons. Provide guidance and knowledge regarding drug information for all other health and medical professionals. Practice in accordance with federal, state and local laws and in compliance with the requirements of the Texas State Board of Pharmacy and Seton regulations and code of conduct. Abides by all regulations and policies for Controlled Substances at all times. Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies. Coordinates and supervises drug distribution, including outpatient prescriptions, investigational drugs and sterile products. Review individual patient medication orders for accuracy, safety and effectiveness and takes corrective action as indicated. Promote cost effective drug therapy in accordance with departmental initiatives and policies. Consistently completes all pharmacy initiatives including, but not limited to, dose optimization, therapeutic interchange, ADR reporting, anti-coagulation monitoring, vancomycin and amino glycoside monitoring, target drug monitoring, and drug use evaluation. Provide patient counseling efficiently and effectively when required. Properly documents all clinical interventions and other information as required by law and departmental policies and procedures. Assist with academic functions such as serving as a preceptor for pharmacy students. GOALS / OUTCOME-BASED QUALITY Participates in the department's Medication Access and Adherence Survey (MAAS) tool in cooperation with the hospital's efforts to drive down preventable 30-day readmissions. Minimum expectations of position (score of 2) are: The pharmacy, as a whole, applies tool and conducts survey to at least 70% of all inpatient admits per year (excludes OB patients). The pharmacist, individually, conducts the surv Qualifications POSITION QUALIFICATIONS: A. Education/Skills/Experience: PharmD OR BS in Pharmacy Degree from an accredited college of pharmacy or equivalent EPGEC (NABP) Prefer hospital experience to include automated order entry skills, IV admixture service to include chemotherapy and TPN compounding, neonatal order entry is a plus, and experience in clinical dosing of antibiotics and anticoagulation monitoring. Good leadership skills are required. Ability to communicate effectively with patients, physicians, and other allied health professionals is required B. Licenses, Registrations, or Certifications: Licensed Pharmacist with Texas State Board of Pharmacy *or* Recent graduate of an accredited pharmacy college and applied to the board to take the NAPLEX and Texas Jurisprudence Examination (extended intern), with completion and receipt of Pharmacist license from Texas State Board of Pharmacy expected within 120 days Completion of 20 hours in a recognized course in an accredited college of pharmacy or course sponsored by an ACPE accredited provider in Sterile Admixture Compounding is required BLS through the American Heart Association required
