Job Summary and Qualifications

POSITION SUMMARY:

Responsibility includes the provision of clinical services and the coordination of care for transplant and living donor patients. Provides education and clinical support for staff in the Transplant Center and in the hospital. Works cooperatively and effectively as a member of the Multidisciplinary Care team.

POPULATION SERVED: ESSENTIAL FUNCTIONS:

All employees, staff of LPMC in both clinical and non-clinical departments, including patient/families, physicians, volunteers, agency, contract, students and the community served.

ESSENTIAL FUNCTIONS:

* Knowledgeable with respect to anti-rejection medication.

a. Drug - drug interaction.

b. Drug - food interaction.

c. Contraindications.

d. Adverse effects (side effects).

* Provide teaching opportunity to small individualized groups; or large more formal class settings.
* Coordinate with outside vendors to acquire "brand name drugs" for patients in preparation for discharge from the hospital after transplantation.
* Provides power point presentation on effects of immunosuppressant medications and transplantation.
* Meets with patients and families in the pre-transplant setting to review and provide a broad overview of all the different types of drugs used in renal transplant medicine in addition to anti-rejection drugs.
* Knowledgeable with "medactionplan.com" website. Able to provide the patient and family a complete list of their specific medication along with e-mail and or telephone alerts to remind patients to take their meds in addition to weekly and monthly medication checklist.
* Meets with patients and families during their hospitalization to educate them regarding the patients specific drug therapy to include drug acquisition, generic substitution, medication storage and medication refills. To include demonstration and return demonstration on the proper filling of a one month supply of medication in the medication tray provided by the hospital.
* Follows with each post transplant patient at least once in the transplant clinic setting to review any concerns the patient or family may have. Is available as needed for continued support.
* Works with the multidisciplinary team to address any patient concerns and issues. Participates in selection committee.
* Acts as the primary resource to the hospital P & T committee
* Documents all activity in patient record.
* Completes other duties as assigned.
* Documents appropriately in the patients medical record all interventions and recommendations to determine candidacy for kidney transplant.
* Demonstrates professionalism, teamwork, and initiative in working with others while maintaining the ability to self-schedule for dietary evaluations.
* Supports the mission and vision of the Las Palmas Medical Center and demonstrates excellent customer service at all times.
* Understands and committed to helping the Las Palmas Medical Center and its associated facilities achieve its goals.
* Must lean and comply with Las Palmas Medical Center facility safety policies and rules; must use appropriate safety equipment and procedures at all times; must immediately report all unsafe conditions to Transplant Clinic Director; must be familiar with all safety features of equipment, tools or materials encompassed by job duties; and must check with Transplant Clinic Director (prior to job performance) if there is a question as to safe procedure to be used for any job function.
* Adheres to the LPMC Code of Conduct and demonstrates ethical behavior as expected by our patients, colleagues and community. Understands the personal obligation to report any activity that appears to violate applicable laws, rules regulations or Code of Conduct itself.

What qualifications you will need:

REQUIRED LICENSURE/CERTIFICATIONS

* Current license to practice pharmacy in the State of Texas.

REQUIRED EDUCATION/EXPERIENCE

* Graduate of an accredited School of Pharmacy
* PharmD. Degree required
* One year experience as a hospital pharmacist is required
