Your Role With Us:

As a Pharmacist On Call at Salem Health, you will work under indirect supervision, monitor and/or manager the medication therapy of specific hospitalized or ambulatory patients. With your knowledge you will provide information about procuring, storing, compounding, and distributing pharmaceutical products.

Pharmacist On Call implements and provides ambulatory pharmacy, inpatient pharmacy or discharge services as a member of the transition of care, Meds-to-Beds and/or clinic teams under the direction of Director of Pharmacy and/or in coordination with Salem Health Medical Group Providers. This position is responsible for managing the pharmacotherapy of complex patients. Our Pharmacists work independently as well as collaboratively, maintaining a positive, professional demeanor while prioritizing tasks and meeting deadlines. You will be constantly challenged to think creatively about how to drive patient care results, stay updated on current trends and practices, and share learnings with team and other health care colleagues.

Core Functions:

+ Evaluate patient electronic medical records to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medications.

+ Participates in Unit Care Rounds and as required review Pharmacy Technician patient interview summaries to obtain information regarding medication use, medication allergies and sensitivities.

+ Evaluate and verify provider orders for appropriateness of drug, dosage, potential interactions, route of administration, stability and make recommendations for therapy changes as appropriate.

+ Monitor and/or manage the ongoing medication therapy of specific hospitalized or ambulatory patients as assigned to assure optimum therapeutic results in the most cost-effective manner.

+ Communicate with prescribing physicians to resolve problems and document therapeutic interventions while assuming responsibility for accurate and timely drug distribution.

+ Supervises and directs activities of Certified Pharmacy Technicians, Pharmacy Interns and Pharmacy Students as well as Pharmacy Residents.

+ Verifies accuracy of medication preparation and dispensing by pharmacy personnel.

+ Serves as a liaison to one or more nursing units or departments which use/ or stock medications.

Background/Expertise:

+ Doctor of Pharmacy (Pharm D), required.

+ Experience in Hospital Pharmacy Practice preferred.

+ Licensure by the Oregon Board of Pharmacy as a Registered Pharmacist (RPh.) required by date of hire.

+ Residency PGY1 preferred.

+ BLS from the AHA, required within 30 days of hire.

+ ACLS, required within 30 days of hire.

+ Board certification in an applicable specialty preferred.
