POSITION SUMMARY: The Pharmacist provides accurate, safe, efficient, and cost effective pharmaceutical care and drug distribution services.# The Pharmacist oversees medications prepared and processed by Pharmacy Technicians.# The Pharmacist is considered the drug expert and must relate this information to care givers and patients seeking or needing it. # EDUCATION/CERTIFICATION: Bachelor of Science or Pharmaceutical Degree from an accredited School of Pharmacy. Must be a Licensed Pharmacist in the State of CT, or be CT license eligible.# # EXPERIENCE: Previous Hospital or home care experience including practice in intravenous admixtures is preferred but not required. Computer experience and typing skills are required to process medication and clinical orders using a computer keyboard. # # COMPETENCIES: Must be articulate and comprehend written and verbal English communications. Must be able to function efficiently in stressful work environment. Must possess and apply the skills and knowledge necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process. This includes; knowledge of growth and development, ability to interpret age specific data and response to care, and be capable of providing age specific communication. Reviews the patient profile for allergies, interactions, and appropriateness before dispensing and demonstrates consistent accuracy in order entry function, in product labeling and dispensing of medications. Intervenes on all requests for non-formulary medications by directly educating the physician about alternatives available on the ECHN Formulary in a polite and consistent manner.# Documents these discussions. Serves as drug information expert.# Promptly provides accurate drug information to other health professionals and lay persons when requested. Adds to the shared knowledge of the department by participating in communication efforts.# These efforts include additions to the Pharmacy Portal, policy and procedural reviews, statistical analysis, or nursing education efforts. Carefully checks the work of Technicians before initialing labels.# Directs Technicians to perform tasks appropriate to current priorities. Assists pharmacy team members by taking on additional tasks during shifts when staffing is not at optimal levels.# This includes activities normally completed by pharmacy technicians. Carefully identifies, prevents, and follows up on drug related problems.# Accurately communicates unresolved issues to the oncoming pharmacist. Participates in Pharmacy staff meetings, interdisciplinary staff meetings, and medical staff meetings.# Effectively promotes communication of Pharmacy interests at these meetings. Participates in medication use evaluations, records clinical interventions, and adverse drug events. Utilizes time in most effective, productive manner by processing order timely and minimizing order queue build up, and responding to pages quickly. Accurately completes all required documentation on pharmacy records and controlled drug records. Keeps work area, mailbox, and Pharmacy clean and uncluttered. #
