Position Overview

The purpose of this position is to assist clinical staff in providing quality patient care by advising physicians and other clinical staff on the selection, dose, interactions and side effects of medications.

Essential Job Functions

* Review all patient charts for correct medication therapy and documentation; including patient drug allergies, drug/drug interactions with current medications, appropriate medication choice, route of administration and dose
* Conduct weekly on-site visits to each facility
* Conduct monthly unit review of department
* Assist Hospital Administrator to ensure sufficient medication is in stock; including working with the pharmaceutical wholesaler to find alternatives, if necessary
* Review and reconcile controlled substances logs and conduct an annual controlled substance inventory
* Comply with all regulatory agency's rules and regulations as applicable to pharmacy
* Maintain appropriate facility licenses - State Board of Pharmacy, DEA and state controlled substance lincense, if applicable
* Assist clinical staff with drug related information
* Provide on-call 24 hour service to answer questions or assist clinical staff if needed
* Counsel patients on medication therapy, if needed
* Plan, monitor and evaluate drug programs and regimens

Other Job Functions

* Attend staff meetings or other company sponsored or mandated meetings as required
* Travel to all facility locations as required
* Perform additional duties as assigned

Basic Qualifications

* Bachelor's Degree in Pharmacy from an accredited school of pharmacy, required
* License in good standing to practice pharmacy issued by the State Board of Pharmacy, required
* Position requires fluency in English; written and oral communication
