* Reports to the Manager of Beacon Home Care Pharmacy. Responsibilities include the accurate preparation and dispensation of all parenteral and other compounded drug products according to physician orders. Oversees and utilizes the pharmacy support staff effectively.

* JOB SPECIFICATIONS
* Education and Experience
* The knowledge, skills and abilities as indicated below are normally acquired through the successful completion of a BS degree in Pharmacy or a Doctor of Pharmacy degree, licensure or eligibility for licensure in the State of Indiana. A minimum of one year of successful hospital, home care or specialized compounding pharmacy experience is required.

* Knowledge & Skills
* Demonstrates pharmaceutical knowledge of parenteral medication compounding, formulation, stability and incompatibilities for the delivery of parenteral infusion sterile therapies in the home.

* Requires the ability to apply pharmacokinetics to specific patient data to dose various I.V. antibiotics.

* Demonstrates working knowledge of nutrition support therapy and the ability to apply clinical assessments and lab data to formulate patient-specific nutrition solutions.

* Demonstrates pharmaceutical knowledge of specialized, (non-sterile), medication compounding, formulation, stability and incompatibilities for delivery of product to human and other animal patients.

* Demonstrates the analytical skills necessary to detect potential drug problems; also assists other healthcare providers in appropriate drug dosages and selection.

* Requires the interpersonal and communication (both verbal and written) skills to interact effectively with patients, caregivers and healthcare personnel and provide medication and supply education.

* Demonstrates working knowledge of clinical drug management.

* Requires the ability to assess insurance information for reimbursement and to surmise covered medications and supplies.

* Demonstrates the ability to use computerized drug information systems for prescription processing.

* Requires the ability to manage time effectively in order to fulfill practice responsibilities so that priority is placed on the delivery of pharmaceutical care.

* Requires the ability to coordinate patient medication and supply deliveries utilizing appropriate personnel.
