Responsibilities

The Pharmacist interprets, monitors and processes physician prescriptions and medication orders. Acts as a drug information resource to patients, medical staff, nursing staff and ancillary department personnel and intervenes to clarify or promote rational medication use. Compounds and dispenses prescribed medications and other pharmaceuticals for patient care by performing the related duties. Processes and reviews pertinent information and medication orders issued by authorized prescribers, into the Meditech computer system. Monitors the orders for accuracy, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities. Provides drug information upon request. Is knowledgeable in all various aspects in Pyxis (Connect, Anesthesia, 3500 and Profile). Compounds medications using standard formulas and processes, such as weighing, measuring and mixing ingredients. Ensures the maintenance and proper operation of equipment and supplies in the work areas. Is familiar with the proper disposal of medications and hazardous wastes normally found in the pharmacy. Demonstrates ability to read, analyze and interpret pharmacy-related journals. Demonstrates ability to respond to common inquiries or complaints from customers, regulatory agencies or members of the hospital staff.

Qualifications

BS in Pharmacy from an accredited College of Pharmacy required. Pharm D. preferred Must hold a current and valid New York State License. Hospital experience is preferred but not essential. Must have excellent communication skills in English. Must have the ability to work well with others. Must be familiar with computers and have a good working knowledge of a clinical information system.
