Description
Job Description As a Pharmacist, you will contribute to the quality and effective operation of the department. To help achieve our mission, you will ensure safe, appropriate and cost effective medication therapies for patients according to established policies, procedures and protocols. Your ability to oversee all department activities pertaining to the preparation and dispensing of pharmaceutical preparations, in accordance with federal, state and local laws and in compliance with the requirements of the Virginia Board of Pharmacy, is of vital importance. Demonstrating proficiency in information systems (e.g. clinical information systems, automated dispensing systems and vault management systems) is required. Job Responsibilities Completes and documents inspections of all assigned medication storage areas on a monthly basis. Identifies and replaces outdated/unusable drugs as documented on unit inspection forms. Answers the telephone in a courteous and timely manner. Responds or redirects requests as appropriate. Supervises and directs pharmacy support personnel. Verifies the daily activities of Pharmacy Technicians. Participates in the performance appraisal of Pharmacy Support personnel. Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures Pharmacy services are provided in a timely manner as evidenced by lack of complaints. Participates in department quality programs as evidenced by documentation of meeting attendance and reports produced. Orders items that fall below par levels or special ordered items. Demonstrates knowledge of vendor order and return procedures for outpatient Pharmacy areas as evidenced by a lack of stock outs and documentation. Keeps Pharmacy areas and equipment clean, neat and well organized. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose documented by intervention reports. Detects and reports suspected adverse drug reactions accurately and in a timely manner by documentation of Adverse Drug Reaction reports. Sustains the formulary by minimizing non formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection by processing non-formulary drug requests and documentation in clinical intervention logs. Provides clinical consultation and clarification to practitioners and patients. Suggests appropriate cost effective therapeutic alternatives to medical staff as needed. Weighs, measures and prepares medication products by utilizing appropriate tools and equipment. Interprets medication orders and transcribes to computerized patient medication profiles accurately. Maintains accurate and complete patient medication profiles as evidenced by a lack of error reports. Accurately compounds and dispenses pharmaceuticals including sterile, chemotherapy and parenteral nutrition products as evidenced by compounding logs. Issues controlled substances to patient care areas and maintains records as required by law. Takes ownership for daily department operations in the absence of the manager or supervisor in accordance with established procedures. Generates reports in accordance with established procedures. Performs minimal troubleshooting as evidenced by resolution of problems. Demonstrates knowledge of clinical modules necessary to provide appropriate patient care. Implements and/or describes backup plan in event of power/communications failure in accordance with established procedures. Performs file backup in outpatient Pharmacy.
