Description

Exciting and challenging Clinical Pharmacist position available at Critical Access Hospital in a beautiful Vermont Hamlet. Join a progressive and innovative team where your individual clinical skills will grow and sharpen as you are exposed to several inpatient settings. Candidate will assume a variety of clinical and distributive functions. The pharmacist will be responsible as a proactive health care team member to identify, resolve and/or prevent potential and/or actual drug related problems.

Opportunity exists to work in our innovative outpatient mental health MTM program. It is the only one of its kind in the state of Vermont. This is a very exciting opportunity for the right candidate. Must be BCMTMS or willing to complete certification to participate.

Essential Functions

Essential functions are those tasks, duties, and responsibilities that comprise the means of accomplishing the job's purpose and objectives. Essential functions are critical or fundamental to the performance of the job. They are the major functions for which the person is held accountable. Following are the essential functions of the job.

1. Oversee all activities of the department pertaining to the preparation and dispensing of pharmaceutical preparations in accordance with federal, state and local laws and in compliance with the requirements of the state's Board of Pharmacy

1.1 Prepares medication products; non-sterile and sterile products, chemotherapy and parental nutrition products accurately as evidenced by compounding log

1.2 Interprets medication orders, transcribes and verifies in computerized information system accurately as evidenced by lack of error reports

1.3 Issues and checks controlled substances to patient care areas and maintains records as required by law as evidenced by count discrepancies and findings from periodic audits

1.4 Responsible for daily operations in the absence of Pharmacy Director in accordance with established procedures as evidenced by number of complaints received

1.5 Reviews and checks packaging, cart fills, IV batches, chemotherapy agents, Total Parenteral Nutrition

1.6 Support the maintenance of appropriate levels of floor stock medications to all patient care areas including but not limited to automated dispensing units, code carts, FQHCs, Windham Center, EMTs

2. Ensure safe, appropriate, cost effective medication therapies for patients according to established policies, procedures and protocols

2.1 Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose documented by intervention reports

2.2 Identify and minimize actual or potential medication-related problems by reviewing the patient's medication profile, medical history, allergy information, and laboratory values, as evidenced by clinical intervention log

2.3 Sustains the formulary by minimizing non-formulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection

2.4 Provides accurate, evidence based drug information to staff, physicians, nurses, and other health care providers, as evidenced by clinical intervention log

2.5 Pharmacokinetics dosing and monitoring of aminoglycosides, vancomycin, anticoagulation and other medications per established protocols and/or requests from providers. Document the intervention on clinical intervention log

2.6 Under the direction of Pharmacy Director, create clinical pathways, protocols, policy and procedures

2.7 Provides clinical consultation and clarification to practitioners and patients. Suggests appropriate, cost effective therapeutic alternatives to medical staff as needed, as evidenced by clinical intervention log.

2.8 Provide medication therapy related in-services to nursing, physicians and other health care providers twice annually

2.9 Detects and reports via SEM suspected adverse drug reactions (ADR) and adverse drug events (ADE) accurately and in a timely manner

2.10 Identify opportunities to reduce the cost of therapy for patients and the hospital

2.11Under the direction of Pharmacy Director, create Drug Utilization Evaluation (DUE) criteria, perform data collection, analyze and summarize the findings and present to medical staff and other health care providers as appropriate

2.12 Participate in rounds

2.13 Perform medication education to patients, medication reconciliation, and other clinical programs

3. Contributes to the quality and effective operation of the department

3.1 Supervises and directs pharmacy technicians and other support personnel. Assist in training technicians and directing work and addressing complaints and resolving problems

3.2 Participates (in conjunction of Pharmacy Director) in the performance appraisal of pharmacy technicians when appropriate

3.3 Works independently with minimal supervision. Organizes and prioritizes work assignments. Ensures pharmacy services are provided in a timely manner as evidenced by lack of complaints

3.4 Participates in department quality programs as evidenced by reports produced

3.5 Up-to-date with policies and procedures

3.6 Participate in quality improvement projects as assigned

3.7 Participate in committee as assigned

4. Proficiency in information systems including clinical information system (CPSI, including charlink), automated dispensing system (Pyxis), CII safe, Smart Pump library set up and other clinical systems

4.1 Able to generate reports

4.2 Able to do minimal troubleshooting as evidenced by resolution of problems

4.3 Knowledgeable on those clinical modules necessary to provide appropriate patient care as evidenced by lack of error reports

5. Other duties as assigned.

Requirements

* Must be fully vaccinated against COVID 19 virus and provide required vaccination documentation unless approved for an exemption prior to starting employment. *

Minimum Education

§ Graduate of an accredited school of pharmacy with a Bachelor of Science degree.

Minimum Work Experience

§ Previous experience as a registered pharmacist in a hospital preferred.

Required Licenses/Certifications

§ Vermont pharmacy license or eligibility for licensure by examination or reciprocity.

Required Skills, Knowledge, and Abilities

§ Excel in Microsoft office suites such as Word and Excel.

§ Recognizes and reports practice deviations to improve the medication system.

§ Communication with pharmacy, nursing, medical staff and other health professionals (within the hospital and community) will be significant.

§ Must be able to demonstrate the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety and related criteria, appropriate to the age of the patients served. The skills and knowledge needed to provide such care may be gained through education, training or experience.
