**Position description**

The Pharmacist is accountable for filling and dispensing prescriptions, providing patient's pharmaceutical care for prescription dispensed, performing patient education and counseling, monitoring drug therapy and performing supervisory functions for assigned ancillary personnel and students and anticoagulation therapy. The Pharmacist also serves as a resource for drug information.

**Qualifications**

Graduate of an accredited College of Pharmacy. Doctor of Pharmacy (Pharm.D.) degree or equivalent experience. Equivalent experience includes at least two years of relevant pharmacy practice experience. Computer experience. Commitment to excellence, accuracy, attention to detail and teamwork. Ability to communicate effectively, work collaboratively with others, organize time well, solve problems, and work independently with minimal supervision.

**Additional qualifications**

License to practice Pharmacy in the State of Wisconsin or eligibility for licensure.

**License or certification**

Currently a registered pharmacist in the State of Wisconsin.
