Job Summary

Review prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their suitability. Provide information and advice regarding drug interactions, side effects, dosage, and proper medication storage. In the hematology/oncology pharmacist role, the successful candidate will review and facilitate inpatient and outpatient oncology medication management in an interactive, interdisciplinary team environment. As part of day-to-day activities, this pharmacist will provide drug information, education, cost containment and clinical services to our hematology/oncology medical staff. The pharmacist will also support transitions of care for adult and pediatric oncology patients undergoing treatment in the inpatient and outpatient settings. The position includes decentralized coverage with team rounding and supports clinical activities such as pharmacokinetics, TPN monitoring and consultation, anticoagulation review, renal dosing, IV to PO adjustments, medication profile review, and antimicrobial stewardship for an oncology patient population. The position includes an every 4th weekend rotation as well as cross training and rotation through the central pharmacy / IV room. This position will also be eligible to participate in committee work and projects as needed.

A successful candidate for this role will have experience and advanced training in the practice of oncology/hematology pharmacy. The completion of a PGY-2 in oncology or Board Certification in Oncology is preferred. Experience with pediatric oncology or bone marrow transplant pharmacy services is favorable.

Sanford Health is a teaching hospital where the pharmacist will have the opportunity to serve as a preceptor for our PGY-2 Oncology Pharmacy Residency program, as well as for our PGY-1 residents, PGY-2 residents, non-traditional residency program and Advanced Practice Pharmacy Experiential students.

Sanford Pharmacy Services are available 24 hours per day, seven days per week in the inpatient setting. The pharmacy team is involved in the inpatient and outpatient departments and work under established P&T protocols as well as collaborative practice agreements. Pharmacy services at Sanford Medical Center also include involvement in cardiology, dialysis, emergency room, general medicine, intensive care, medication reconciliation, medication delivery to bedside, oncology, OR pharmacy, transplant, and an antimicrobial stewardship program.

Qualifications

In a medical center or institutional setting: Graduate of an accredited School of Pharmacy. Must have a thorough knowledge of drugs and their actions; knowledge of state and local regulations. Hospital experience or residency preferred.

License must be in good standing by the State Board of Pharmacy in state(s) of practice and/or possess multi-state licensure privileges as required by position.
