Description:

This position is for an ambulatory pharmacist that will be working in a physician office practice. This pharmacist will be co-managing patients with complex chronic diseases as well as medication and anticoagulation management. The ambulatory pharmacist will serve as a resource to patients, caregivers and other members of the patient's care team.

Position Summary:

Contributes to the care of the patient by developing pharmacotherapeutic regimens and counseling utilizing pharmacology, pathophysiology, pharmacokinetics, and interpersonal communication skills. Provide clinical pharmacy and drug information services to serve hospital staff and collaborate with physicians. Engages in quality improvement and professional growth to meet hospital, departmental, and personal goals. Compound, package, and dispense medications and IV solutions and preparations as ordered. Assist in the supervision of pharmacy technicians and interns in the operations of the pharmacy. Acts as a resource to students of various health disciplines.

What the Pharmacist will do:

* Collaborate with other healthcare providers as well as providing direct patient care.
* Supervise Pharmacy Interns and Technicians to ensure proper training and job performance according to established policy, procedure, and protocols.
* Work with patients to improve adherence and evaluate medication regimens

What the Pharmacist will need:

* BS degree in Pharmacy. Licensed to practice in the State of Michigan. Advanced degrees (i.e., Doctorate of Pharmacy) and residency training preferred.
* Demonstrated commitment to hospital pharmacy practice, as demonstrated by recent (within 5 years) employment in hospital practice or advanced clinical pharmacy work.
