We Are Hiring We are looking for a dedicated and knowledgeable Pharmacist (Clinical Specialty) to join our team and work PRN, Days (8a-4:30p), Monday through Friday. What You Will Do Assure the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as nuclear medicine, oncology, cardiology. Serve as a subject matter expert in pharmacy discipline and provide guidance across pharmacies and to clinical leadership. Review and assess medication orders to identify, prevent and resolve potential and actual drug related problems. Recommend formulary alternatives when appropriate. Compound and dispense medications for accurate delivery to patients. Provide drug information and education to medical staff and patients. Communicate with physicians and other clinicians as necessary to facilitate care. Oversee technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies. Assist with academic functions such as serving as a preceptor for pharmacy students. May float to provide coverage of pharmacy needs. Provide patient counseling when required. What You Will Need Licensure / Certification / Registration: Pharmacist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date required. Education: Bachelor's degree in Pharmacy or Doctorate of Pharmacy required. Pharmacy residency preferred. Additional Preferences Responsibilities: •Work under a Collaborative Drug Therapy Management Agreement (CDTM), which allows qualified pharmacists within the context of a defined protocol to perform specific patient care functions such as modifying pharmacologic drug therapy regimens; dose, frequency, discontinuation and/or initiation, renewal of existing prescriptions. •Manage chronic disease states for patients seen post-discharge or for long-term chronic disease management •Supervise certified pharmacy technicians completing direct medication assistance activities Qualifications and educational requirements of clinical pharmacists practicing under this protocol include: 1.Licensure: a.Active licensure in good standing with the Illinois Department of Financial and Professional Regulation (IDFPR) 2.Training: a.Completion of an American Society of Health System Pharmacists (ASHP) accredited PGY-1 Residency and ≥ 1 years of experience in ambulatory care practice OR Completion of an ASHP accredited PGY-2 Residency in one of the following specialties: Ambulatory Care, Pharmacotherapy, Family Medicine, Cardiology, Internal Medicine, Geriatric, Nephrology, or Transitions of Care OR Doctorate of Pharmacy (PharmD) or Bachelor of Pharmacy (B.S.
