Job Summary and Qualifications

POSITION SUMMARY

The Pharmacist Clinical Specialist serves as the expert medication management team member; in collaboration with medical, nursing, and pharmacy staff. The position ensures appropriate pharmacotherapy in their specialized units. The Pharmacist Clinical Specialist implements and monitors clinical protocols and guidelines, and enhances the delivery of evidence based medicine. Provides medication distribution support, order entry, clinical intervention, research, performance and quality improvement, and representation at the medical center committees and meetings.

PRINCIPAL RESPONSIBILITIES

  • Provide leadership and comprehensive pharmaceutical care to patients within their specialty.
  • Communicate effectively between providers, nursing, pharmacy and related staff in providing pharmaceutical care.
  • Collaborate with others to resolve medication-related issues, including the development of interventions, monitoring and documentation of actions and outcomes.
  • Promote and demonstrate pharmaceutical care that optimizes safety, efficacy and cost; including monitoring for adherence to the formulary, criteria-for-use/restrictions, goals/desired outcomes. Develop, participate, and lead targeted programs and medication use evaluations to improve outcomes while being cost-effective (e.g. Antibiotic stewardship)
  • Educate prescribers and other healthcare professionals as appropriate.
  • Provide training and education to pharmacy students, pharmacy residents, pharmacists and pharmacy technicians as appropriate on pharmaceutical care and disease state management.
  • Demonstrate understanding of HCA's and "Patients First" safety initiative by strict compliance to all safety protocols and procedures.
  • Foster development of non-specialist pharmacist knowledge, skills and ability in providing pharmaceutical care to patients in specialists' area. (e.g. case review with pharmacist)
  • Play an active role in supporting facility, state, federal regulatory and accreditation (e.g. TJC, AHA Stroke Accreditation) activities and initiatives.
  • Performs other duties as assigned or required.

JOB REQUIREMENTS:

LEVEL OF FORMAL EDUCATION REQUIRED:

Required: PharmD or BS Pharm degree

LICENSED/CERTIFICATION REQUIRED

  • Current Texas Pharmacist License or within two months of hire.
  • Certification in sterile products preparation or complete within two months of hire.

EXPERIENCE REQUIRED:

  • Required: (PGY1) Pharmacy Practice Residency or commensurate Board
  • Certification or commensurate experience
  • Minimum one year pharmacist experience
  • BLS, ACLS, PALS, and ENLS certifications (if applicable to clinical
  • specialty) within six months of employment.
  • Preferred: (PGY2) Specialized Residency in area of specialty
