Description
Graduate of accredited college of Pharmacy, SC license or Board eligible. Clinical pharmacy background. Responsibilities include, *Therapeutic drug monitoring on nursing unit, pharmacokinetics and drug information to clinicians. *Documenting daily interventions *Completing reports of financial data obtained through interventions *Supervises and/or prepares medication orders and prescriptions with dosage appropriate for the age, weight, disease state, etc of the patient *Participates in performance improvement and medication use evaluation activities *Participates in adverse drug reaction error reporting programs *Supervises and/or fills medication carts/AcuDose *Ensures the accurate and aseptic preparation of IV admixtures, cancer chemotherapy agents and total parenteral nutrition solutions *Any other associated tasks as assigned by the pharmacy management staff
