SUMMARY OF POSITION # The primary purpose of this position is to be responsible for oversight of daily operations; interprets physician#s prescriptions and medication orders; acts as a drug information resource to patients, physicians and staff; compounds and dispenses prescribed medications and other pharmaceuticals; monitors potential for drug/drug interactions; works with staff to achieve medication and reconciliation at each level of patient treatment; collects and interprets performance improvement data; fills employee prescriptions. # RESPONSIBILITIES # EXPECTATIONS # Reviews prescriptions issued by physicians and other authorized prescribers to assure accuracy and appropriateness. Monitors patient profiles for accuracy, drug allergies, drug interactions, duplicate therapy, rational drug therapy and incompatibilities. Optimizes patient safety and reduces the likelihood of errors by maintaining a work environment that is conducive to safe medication preparation. Compounds sterile products or admixtures competently and safely and works in accordance with USP 797 standards. Monitors personnel performance of admixing, packaging and labeling pharmaceuticals. Serves as a resource to staff, nursing, physicians and patients for drug information and consultation to patients and/or caregivers when needed. Enters data into the AcuDose system to maintain the accuracy and comprehensiveness of the hospital formulary and delivery system. Actively participates in the Pharmacy and Therapeutics Committee when needed. Maintains compliance with Federal and State regulatory standards, The Joint Commission, and compliance with 340B program. Provides pharmaceutical care to all age groups. Counsels new patients on admission and performs discharge education on newly prescribed medication. Performs medication reconciliation for patients in all areas of the hospital. Establishes positive collaborative relationship with administration, physicians, nursing and hospital employees. Delivers timely internal and external customer service in a cooperative, professional and respectful manner; performs all job functions with integrity. Provides oversight of the employee prescription program. Provides Medication Therapy Management services to employees and other Aultcare members. Participates in new employee orientation and continuing education of employees regarding medication management. Fosters an environment of learning and education; Willingness to serve as a preceptor for NEOMED and Ohio Northern pharmacy students. JOB REQUIREMENTS # # # BS, PharmD from an accredited college of pharmacy. BLS certification. Minimum two years# experience in healthcare related field; hospital experience preferred. Current Ohio license in good-standing with the Ohio State Board of Pharmacy as a Registered Pharmacist or have the ability to reciprocate within six (6) months. Demonstration of strong leadership skills including critical thinking, decision analysis, collaboration, change management and strategic planning. Ability to set priorities and reprioritize as needed. Ability to use conflict resolution and negotiating skills to resolve problems. Organized, analytical, and efficient in performance of duties. Computer skills with Word, Excel, and PowerPoint. Good oral and written communication skills; Demonstration of strong communication skills including educator, presenter and speaker capabilities. Must function independently, have flexibility, personal integrity, knowledge of the importance of maintaining confidentiality, and have the ability to work effectively with all levels#of hospital personnel and other facilities in the foundation.
