The position is located at the CHI Health Pharmacy Anticoagulation Clinic. The CHI Health Pharmacy Anticoagulation Clinic manages patients who are on warfarin and need regular monitoring and dosing adjustments.

Responsibilities

We can't begin to describe how much we appreciate our Pharmacists. Not only are they responsible for making sure our patients receive their prescribed medication, but they also have to work with those patients and their families on how and when to take them correctly. They sometimes have to get creative. They work for our patients, from reconciling medications from different healthcare professionals to talking to patients about medication discounts and coupons. Must possess strong knowledge of Pharmacy Services; knowledgeable and skilled in current practice concepts of nursing home, community pharmacy service, and home I.V. Therapy; strong knowledge of applicable laws, regulations, guidelines and professional standards; strong project management skills; good strategic planning skills; strong change management skills; must demonstrate personal traits of a high level of motivation; team orientation; professionalism; trust; and place a high value on treating others with dignity and respect.
