Description
The Ambulatory Clinical Pharmacist provides professional pharmaceutical services in the ambulatory setting to patients, healthcare providers, and clinic staff. The Ambulatory Clinical Pharmacist ensures optimal pharmaceutical therapy outcomes and prospectively develops pharmaceutical plans and medication therapy education for each patient. All activities are done in accordance with established policies, procedures, and regulations with the health and welfare of the patient paramount.

This position has a primary focus in the Endocrinology Clinic with time spent in transitions of care and inpatient services as needed. Experience with clinical program development and specialty pharmacy coordination within an ambulatory care setting are encouraged to apply. Ideal candidate must be patient-centered, innovative, adaptable to change, and autonomous as we are a dynamically growing ambulatory service!

Qualifications

+ Graduate from an accredited ACPE school of pharmacy is required

+ Previous experience with pediatric patients in a hospital setting is required

+ Current license or license eligible to practice as a registered pharmacist in Texas

+ Current Pharmacy preceptor license or eligible. Must be obtained within 90 days of employment

+ CPR - BLS is required. PALS required for critical care areas only

+ Satisfactory completion of a post-graduate year 1 residency or 3 years experience in a comparable health care setting preferred, not required
