Full Time (hours may vary)

JOB DUTIES - It is expected that all duties and responsibilities of this position will be performed in a manner consistent with the mission and core values.

* Complies with Kentucky and Federal Pharmacy Laws, Joint Commission standards, and National Patient Safety Goals regarding medication management.
* Serves as the drug information expert and resource regarding pharmaceuticals for the health system and the community it serves.
* Prioritizes workflow by patient impact.
* Interprets and reviews medication orders received for correct drug, dosage, route, frequency, allergies, therapeutic duplications, interactions, contraindications, and potential adverse effects.
* Oversee dispensing of pharmaceuticals, reconciliation of controlled substances, management of expirations and recalls.
* Supports and documents completion of clinical and cost savings initiatives, therapeutic drug monitoring, and appropriate formulary utilization
* Demonstrates a commitment to safety through well documented reporting of safety events.
* Demonstrates supervisory skills necessary to plan organize and direct activities in the pharmacy in a cost effective manner.
* Provides technician oversight and assumes role of supervisor in routine daily activities.
* Effectively utilizes pharmaceutical references.
* Has a working knowledge of pharmacy operations including: pharmacy operating system, use of patient record, purchasing, inventory and formulary management, automation, packaging, USP 797, USP 795, pharmaceutical waste, Joint Commission standards.
* Proficient in pharmaceutical calculations.
* Promotes engaged learning environment for pharmacy students.
* Follows hospital and departmental policies and procedures.
* Adheres to medical ethics and has concern for the welfare of the patient, pharmacy and institution.
* Willingly accepts other duties as assigned.

Clinical Pharmacy Services

Retail Pharmacy

Pharmacist I: Can function independently in all aspects of central pharmacy tasks or one de-centralized pharmacy service.

Pharmacist II: Can function independently in all aspects of two or more de-centralized pharmacy services as well as all central pharmacy tasks.

Pharmacist III: Primarily work third shift (2).

QUALIFICATIONS AND ACCOUNTABILITY -

* Kentucky Board of Pharmacy license in good standing
* Doctorate of Pharmacy degree preferred
* Hospital Pharmacy experience preferred
* ACLS preferred
* Proficient in clinical skills for patients in the following age categories:

Neonate-0 to 4 weeks, infant-4 weeks to 1 year, child-1 to 12 years, adolescent-13 to 17 years, adult-18 to 70 years, and geriatric-70 years and above.

* Attentive to detail.
* Ability to efficiently organize and prioritize workflow for timely completion.
* Flexibility to adaptability to interruptions in workflow and continuous change.
* Critical thinking skills to identify potential process failures and solve complex operational and patient care problems.
* Interpersonal skills necessary to interact with all levels of hospital personnel.
* Demonstrate customer service priority to anticipate and exceed expectations.
* Trustworthy and accountable.
* Engaged in continuous growth and development.

Supervised by the Pharmacy Director and Pharmacy Manager. Performance evaluations are conducted at the end of the three-month probationary period by the director with input from peers and thereafter on a yearly basis. Successful completion of competency required to remain in role.
