Ensures the safe, efficacious and cost effective preparation and use of medications. Interacts regulatory with patients, ensures appropriate medication reconciliation. Serves as facilitator for pharmaceutical care team; participates in ongoing education of team members and patients: coordinates quality improvement initiatives of team; facilitates formulary management; works to ensure the safe, efficacious, and cost-effective use of medications and preparation of prescriptions in assigned patient populations.

1.Education and Training
*Bachelor's Degree or PharmD. in Pharmacy
*Current Licensure in State of Ohio
*Completion of residency training in Pain/Ambulatory pharmacy practice or three years of experience in a pharmaceutical care role in an ambulatory setting.

One year of experience working in a Hospital Pharmacy preferred; Pain Management experience preferred.

3.Other Skills, Competencies and Qualifications
*Ability to effectively communicate verbally and in writing with pharmacists, physicians, nurses, technical staff, students, pharmacy resident(s), patients and other healthcare
*Ability to work with others on interdisciplinary or multidisciplinary teams in providing safe
and cost-effective pharmaceutical care to patients.
*Ability to assist in achieving compliance with accreditation, legal, regulatory and safety requirements related to pharmaceutical products.
*Ability to provide in-service and/or continuing education on drug-related topics to pharmacist, physicians, nurses and other healthcare professionals.
*Ability to investigate, formulate and provide appropriate timely responses to drug information questions requested by pharmacists, physicians, nurses and other healthcare professionals.
*Ability to direct, manage and instruct pharmacy resident(s) and students.
