The clinical pharmacist will be responsible for providing clinical and dispensing pharmacy services to the patients receiving care in Randolph Hospital inpatient and outpatient care areas. This pharmacist will perform all functions in accordance with applicable federal and state laws and regulations, accrediting agency requirements, hospital policies, and pharmacy practice standards.

Must be a graduate of an accredited School of Pharmacy with a PharmD degree preferred; will consider Bachelor of Science degree with a minimum of five years of clinical experience. Must hold and maintain a current license to practice pharmacy in the state of North Carolina. Must obtain an annual minimum of fifteen hours of continuing education from an approved provider. Must complete all mandatory Randolph Hospital continuing education programs.

Must demonstrate understanding of the composition, properties, pharmacological actions, and clinical effects of pharmaceutical products and delivery systems. Must be able to obtain, assess, and interpret patient- and medication-specific data and use that information to optimize medication therapy. Expectations include ability to develop optimal medication regimens, provide accurate risk/benefit assessments of medication therapies, recognize and initiate risk minimization practices, monitor therapeutic and toxic effects, and provide and implement therapy recommendations to ensure patient safety. Must demonstrate knowledge and skills necessary to provide appropriate medication therapy and delivery methods for neonatal, pediatric, adolescent, adult, and geriatric patients served by Randolph Hospital. Must understand the principles of physiology, growth, and development over the human life span and the impact of these variables on medication efficacy and safety. Must be able to accurately and safely complete all aspects of pharmaceutical compounding, dispensing, and documentation. Must be able to effectively communicate with health care professionals, patients, and caregivers. Must be able to provide accurate and timely medication information. Must be able to evaluate and accurately interpret written and verbal prescriptions from licensed independent providers, determine optimal administration parameters (such as infusion rate, diluent type and volume, etc), establish appropriateness of therapy, and correctly prepare, label, and dispense medications on the basis of such prescriptions. Must be able to demonstrate or develop ability to create an accurate and complete medication profile in the Meditech Health Information System. Must be able (after appropriate training) to operate pharmacy automated systems (Meditech, MedSelect, Centrack, Pyxis Scanner, QS1), provide initial evaluation and response in case of malfunction, and implement back-up procedures in the event of system failure. Must be able to prioritize and manage multiple responsibilities and coordinate pharmacy workflow. Although most scheduled work hours are staffed with more than one pharmacist, the clinical pharmacist must be able to function as the only pharmacist on duty when necessary. Must provide effective supervision of pharmacy technicians and verification of their work product. Previous hospital experience preferred
