I. Primary Function/General Purpose of Position
The Team Member is responsible for providing ethical and professional pharmaceutical care, specific to all ages of patients that service is provided to, within the framework of hospital and departmental policy, professional standards of practice, state and federal laws and JCAHO requirements.
II. Employment Qualifications
· B.S. degree from an accredited school of pharmacy or equivalent.
· License or eligible for licensure in Virginia.
· One or two-years hospital pharmacy experience preferred.
III. Essential Job Functions
· The employee is able to demonstrate general competency in providing Age Appropriate Care as indicated by the ability to plan, provide, and evaluate care based on the physiological, psychosocial, intellectual, and safety developmental needs of the patient.
· Prepares and dispenses medication orders per physician request according to established policies, procedures, and protocols.
· Ensures safe, appropriate, cost-effective medication therapies for patients according to established polices, procedures, and protocols.
· Contributes to the quality and effective operation of the department.
· Maintains competence required for current job title/position.
· Performs other duties as assigned by supervisor.
