Overview
The Oral Anticancer Education Pharmacist is responsible for providing education to patients starting oral anticancer agents in the home setting. The pharmacist will work collaboratively with other disciplines to optimize the pharmaceutical care of these patients and to ensure that patients receive follow-up monitoring to assess adherence and toxicity. The pharmacist will also participate in the development of workflow design, teaching documents, education and/or monitoring tools, and other initiatives that will improve and/or expand the program. In addition, the pharmacist will train new staff to the program and the PGY2 oncology pharmacy residents during their OAT rotation. The oral anticancer teach pharmacist will perform the day-to-day operations necessary to ensure that patients are contacted and taught in a timely manner.
This position is fully remote. Employees must reside in MA, NH or RI.

Responsibilities
The Oral Anticancer Educaiton Pharmacist is responsible for the optimization of drug therapy by identifying, resolving, and preventing drug-related problems and adverse drug events.
Interprets, clarifies, and evaluates oral anticancer prescriptions for completeness and appropriateness Assesses drug therapy for contraindications, interactions, allergies, baseline clinical and laboratory assessments, and adjunct supportive care medications.
The pharmacist provides drug Information, education, and clinical services.
Provides clinical consultation and suggestions to physicians and nurses, regarding appropriate, cost-effective therapeutic pharmacotherapy needs for patients based on the oral anticancer agent prescribed. Communicates any recommended adjustments to the prescription or medication order or patient instruction discrepancies with the healthcare team. Provides comprehensive education to patients initiating oral anticancer therapy
Ensures the daily operations of the Oral Anticancer Teach Program
Implements and coordinates the daily assignments of the OAT activities to ensure all tasks are completed and patients are counseled Follow up on existing patients in queue, work up new patients to add to the queue, check communications from InBasket, voicemails and emails. Troubleshoots any clinical, operational, or technological system issues related to the daily activities Collaborates with other teams involved in the OAT program to maintain an effective workflow
Participates in process and quality improvement
Maintain teaching documents and other education and/or monitoring tools Works collaboratively with OAT pharmacists and other teams involved in the OAT program to assess current processes and identify opportunities for improvement Participation in OAT quality improvement efforts when appropriate
Has working knowledge of and is able to use all technology required to perform functions of daily duties
Adheres to Department Standards/Teamwork/Collaboration
Qualifications
MINIMUM JOB QUALIFICATIONS:
* Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of Pharmacy and must be licensed to practice pharmacy in the Commonwealth of Massachusetts.
* 3 to 5 years of health system pharmacy experience is required as well as a minimum of 2 years of oncology experience or the completion of a PGY-2 oncology residency.
* Patient education/counseling experience preferred.
