Description
This is a clinical supervisor position in a large, modern, active, long term care facility responsible for the day to day clinical and performance improvement activities of a complex pharmacy service. Responsibilities include the immediate supervision of registered pharmacists and ancillary personnel, provision of pharmacy services to the long term care facility for the purpose of achieving definite outcomes that improve the resident's quality of life.

+ Insures the proper maintenance of all LTC pharmacy areas and equipment used in these areas.

+ Participates in the training of new personnel for LTC services, including staff pharmacists, residents, externs and non-professional staff.

+ Participates in the supervision of assigned personnel, professional and non-professional.

+ Participates in all formal training programs of the pharmacy department including the teaching and supervision of students, residents and others.

+ Contributes to the establishment and maintenance of automated systems within the pharmacy department.

+ Maintains awareness of trends in pharmaceutical and related literature by participating in professional continuing education programs.

+ Performs staff responsibilities as required to insure continuity of service such as:

+ filling of bins

+ retrieving STAT orders from the main pharmacy

+ medication order entry in QS1 or similar data base

+ unloading weekly order

+ ability to operate and run Automed if need be

+ Assures the smooth operation of the LTC Pharmacies, including responsibility for overseeing:

+ maintenance of resident/patient drug profiles

+ ordering of drug supplies from the Cardinal Express Ordering system, maintaining all appropriate records and supplies

+ provision of drug information to physicians and nurses including appropriate drug use, interaction and toxicity

+ removal of outdated drugs from all assigned areas

+ inspection of nursing stations and maintenance of records

+ acceptance of all legal responsibilities of "supervising pharmacist" for a long term care facility

+ stocking and updating Pyxis with narcotics, responsible for inventory of the narcotics in the safe

+ Interacts with members of the LTC departments of Nursing and Medicine. This includes direct problem-solving interaction with the staff nurses and physicians regarding policies and procedures and the appropriate use of drugs.

+ Assures maintenance of resident/patient drug profiles, utilizing unit-dose drug distribution and assures all policies pertaining to this distribution are adhered to.

+ Performs Medication Reconciliations for new admissions to the LTC facility, generates interventions when necessary.

+ Distributes Medication Regimen Reviews performed by Elite Consultants to physicians and nursing. Responsible for collecting back the Reviews.

+ Complies with all Department of Health and other governmental agency rules and regulations.

+ Participates in all activities in preparation for inspection by governmental agencies and accrediting bodies.

+ Follows up and resolves all outstanding drug interventions made at the point of entry by the pharmacist.

+ All other duties as requested.

Education:

+ Degree in Pharmacy from an accredited College of Pharmacy

+ An advanced degree (PharmD) in the position-related specialty.

Experience:

+ Minimum two (2) years hospital pharmacy experience.

+ American Society of Health System Pharmacists (ASHP) accredited pharmacy residency and prior supervisory experience are highly desirable.

Licenses, Certifications:

+ Current New York State Pharmacist License and Registration required.

+ An advanced degree (PharmD) in the position-related specialty.

Physical Requirements:

+ Position requires prolonged periods of standing, reaching, walking throughout the working day.

+ Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
