Description
The Medication Reconciliation Technician provides technical assistance with documentation and clarification of medication history lists by conducting patient and/or caregiver interviews to obtain the most current and accurate medication history. physician/nurse medication histories and completes full history documentation in computerized system. Collects necessary data to assist with the transformation of the medication and medication reconciliation process across the continuum of care. information through communication with the providers, patients, and other pharmacies. Works with multiple medication automation systems. Assists in the daily activities of the Pharmacy Department to ensure that optimum pharmaceutical care is provided to all patients. Assists registered pharmacists and other pharmacy technicians in the preparation and dispensing of sterile and non-sterile medication, maintains records and inventory, following procedural work routines, to ensure medications are ready and available for patient administration. Skills and Requirements High school diploma or equivalent required, Associates degree preferred Registered by the Massachusetts Board of Pharmacy and Nationally certified as a Certified Pharmacy Technician (CPT) by the Pharmacy Technician Certification Board required Minimum of 2 year experience working as a technician.# Medication counseling and bilingual candidates preferred.

Clarifies information through communication with the providers, patients, and other pharmacies. Works with multiple medication automation systems. Assists in the daily activities of the Pharmacy Department to ensure that optimum pharmaceutical care is provided to all patients. Assists registered pharmacists and other pharmacy technicians in the preparation and dispensing of sterile and non-sterile medication, maintains records and inventory, following procedural work routines, to ensure medications are ready and available for patient administration.

Skills and Requirements

High school diploma or equivalent required, Associates degree preferred

Registered by the Massachusetts Board of Pharmacy and Nationally certified as a Certified Pharmacy Technician (CPT) by the Pharmacy Technician Certification Board required

Minimum of 2 year's experience working as a pharmacy technician.

Medication counseling experience and bilingual candidates preferred.
