Our Medication Reconciliation Pharmacy Techs assist hospital staff pharmacists in the preparation and dispensation of medications. You will serve a vital part in our pharmacy team by assisting the medication reconciliation pharmacist in ensuring patients have a pharmacy services completed medication history.

  • You will interview patients and family members for the purpose of compiling an accurate and complete list of current medications the patient was taking prior to admission to the hospital, to include all prescription, non-prescription, and investigational medications
  • You will contact the patient's community pharmacy, physician's office, or other healthcare facility for clarifications if needed
  • You will accurately update medication history information in the electronic health record
  • You will correctly and completely document interventions made to the medication history in the electronic medical record prior to the end of each shift worked

REQUIREMENTS

  • High School Diploma or equivalent
  • Licensed Pharmacy Technician through the Virginia Board of Health Professions
  • Certified by the Pharmacy Technician Certification Board (PTCB)
  • Previous inpatient pharmacy experience preferred
  • Full Time
  • Day Shift to include rotating weekends and holidays
