Seeking a Medication History Specialist with a background as a retail pharmacy technician to join WellSpan's Emergency Medicine Department Job Description:

Under the direct supervision, interviews patients and reviews medication-related documentation for the purpose of compiling a medication history for patients being admitted to York Hospital. Compiled history information is entered into the patient's electronic health record for the purpose of facilitating medication reconciliation by the admitting provider. May perform a variety of functions which impact the medication reconciliation process and patient care.

Duties and Responsibilities Essential Accountabilities:
  1. Collects an accurate best possible medication list of the patient's home medications in anticipation of hospital admission.
  2. Gathers information for best possible medication list from various resources (patient interviews, physician offices, community pharmacies, historical encounters, extended care facilities, or other acute care facilities) to ensure a complete and accurate entry into the hospital's electronic health record.
  3. Effectively communicates with other health care professionals to prioritize patient care and identify patient specific needs.
  4. Accurately enters the home medication list into the Hospital's information system for utilization by the provider responsible for admitting the patient.
  5. Updates the medication list in a timely manner so it can be used as the foundation for the patient's admission medication orders.
  6. Directs any unresolved issues to the admitting provider.
  7. Gathers medication information for patients in the ED and those being directly admitted to the floor.
  8. Greets, directs and assists patients, visitors, physicians and ancillary department staff.
  9. Answers telephones using personable etiquette. Answers inquiries by providing accurate information or forwarding callers to the appropriate resource. Identifies emergency messages, directs information to the appropriate personnel and maintains confidentiality.
  10. Communicates delays/difficulties in reconciliation process with appropriate disciplines.
  11. Assists with care and maintenance of department equipment, hardware and supplies.
Qualifications:
  • Minimum Experience:
    • 3 - 6 months
  • Minimum Education:
    • High School or G.E.D.
  • Preferred Field Of Expertise:
    • Experience with medication names, their uses, doses and routes of administration
  • Required Courses:
    • 4 weeks hands-on training in the department; Medical Terminology (Optional)
  • Skills:
    • Excellent communication/interpersonal skills; excellent computer skills and the ability to use various software applications at once

    • Effective time management skills with the ability to prioritize tasks
