Internal Medicine/Medication Safety Clinical Pharmacy Specialist
Location: Baton Rouge, LA
About the position:
Our Lady of the Lake Regional Medical Center is seeking a passionate and highly motivated Internal Medicine/Medication Safety Clinical Pharmacy specialist to join our advanced clinical pharmacy team. This position will work in collaboration with a team of 4 Internal Medicine Clinical Staff pharmacists and will round with one of the 4 Internal Medicine teaching services. Additionally, this pharmacy specialist will serve as a leader in medication safety in collaboration with pharmacy department leadership and other clinical pharmacists.
Daily roles and responsibilities:
* Evaluate, design, implement, monitor, and modify medication regimens based on standards of care and individual patient needs
* Review internal medicine service patients and discuss with physicians through both formal and informal patient care rounds
* Serve as an internal medicine content expert for fellow pharmacists, physicians, and other healthcare providers
* Complete therapeutic drug monitoring consults including vancomycin and aminoglycosides
* Utilize hospital-approved protocols for automatic IV to PO and renal dosing protocols
* Utilize the Epic® electronic medical record to document in the patient care chart as appropriate
* Monitor submission of pharmacy-related medication safety events and provide real-time education and feedback
* Precept pharmacy students and pharmacy residents
Longitudinal roles and responsibilities:
* Perform root-cause analyses of medication safety events to develop process improvement plans
* Assist in the development and maintenance of formulary
* Conduct quality improvement projects to promote the rational, safe, and cost-effective use of medications.
* Collaborate with the rest of the clinical pharmacy team to create guidelines, protocols, and policies
* Present quality and economic outcomes data and findings to local P&T committee

Our Lady of the Lake Regional Medical Center (OLOL-RMC) is one of the largest private medical centers in Louisiana with 988 beds.
